Qualifications

• Bachelor’s degree required
• Must be able to pass a Level 2 background screening
• Minimum of 2 years of experience working with individuals with behavioral health needs and/or developmental disabilities

Position Details

• Initial training will be completed at our office located in Haines City
• Client home visits are required based on individual living settings, with most visits occurring no more than once per month
• Flexible, community-based work environment
• Make your schedule as needed

Responsibilities

• Maintain accurate and up-to-date progress notes and central files within the iConnect system
• Coordinate and monitor services provided by multiple care providers and agencies
• Assist individuals in accessing community resources, programs, and support networks
• Advocate for the rights, goals, and needs of the individuals served
• Build positive relationships with clients, families, and community partners
