Position Summary
The Case Manager is responsible for assisting clients of the Covenant Shelter of New London, Inc. in accessing housing and support services. This role requires close collaboration with other shelter staff and external social service organizations to address the unique needs of homeless individuals and families, with the primary goal of securing and maintaining stable housing for clients.

Key Responsibilities

Resident Support & Advocacy:

Conduct intake and needs assessments with shelter clients to identify housing barriers.

Meet with shelter guests regularly, including weekly meetings and up to two meetings per week as needed.

Develop individualized housing stability plans tailored to each resident’s needs and update on a bi-weekly basis with a major emphasis on housing.

Provide coaching and education on lease agreements, tenant responsibilities, and housing interviews.

Advocate on behalf of clients with landlords, housing providers, and community partners.

Conduct bi-weekly case management groups

Complete 30 day reviews, incident reports, housing plans, case file reviews and case notes as needed.

Housing Coordination:

Assist clients with completing housing applications, obtaining required documentation (e.g., ID, Social Security card, birth certificate), and navigating housing processes.

Establish relationships with housing programs, landlords, and community organizations to secure housing opportunities.

Coordinate access to resources, including financial assistance, legal aid, and employment readiness programs, mental health, medical, ect.

Support clients in addressing issues that could threaten housing stability and provide crisis intervention when necessary.

Documentation & Reporting:

Maintain detailed and accurate case files, progress notes, and documentation for all clients on the caseload.

Collect program evaluation data and submit required reports.

Utilize the Homeless Management Information System (HMIS) portal to track client progress and report outcomes.

Collect entrance and exit surveys for all clients

Team Collaboration:

Participate in weekly housing meetings to discuss resident progress and address challenges.

Attend regular case conferences with shelter staff to coordinate services and review clients' needs.

Attend weeklys and bi-weekly Eastern Coordinated access family and individual housing meetings.

Hold monthly meetings with individual guests and families, including those with minor children, to review housing stability goals.

Attend quarterly staff meetings to share updates and collaborate on shelter objectives.

Community Engagement:

Build and maintain partnerships with housing programs, substance abuse services, legal aid, TANF, SNAP, Veterans Services, and other community resources.

Collaborate with external organizations to connect clients with available services and supports.

Attend and support internal workshops with community partners

Other Duties:

Perform additional tasks as assigned by the supervisor to support shelter operations and ensure the well-being of residents.

Supervisory Responsibilities

This job has no supervisory responsibilities.

Required Knowledge, Skills, and Abilities:

Knowledge of supportive housing programs, affordable housing resources, and community services.

Understanding of homelessness, mental health issues, substance use, and trauma-informed care.

Strong organizational, communication, and advocacy skills.

Ability to work independently and manage a caseload in a fast-paced environment.

Sensitivity and understanding when working with vulnerable and diverse populations.

Commitment to equity and inclusivity.

Proficiency in using case management software, including HMIS and overall comfort with computers.

Qualifications

Associate’s degree in social work, human services, or a related field required. Bachelor’s degree preferred.

Minimum of 1 year of case management experience, particularly in a shelter or housing program setting, is highly preferred.

Bilingual (English/Spanish) preferred.

Certificates, Licenses, Registrations

Valid Driver's License preferred, CPR Certification

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
