The Licensed Vocational Nurse (LVN) is part of the CHT clinical team, playing a key role in delivering Covenant House’s life-saving intervention to Houston’s homeless young adults, ages 18 to 24.

The Licensed Vocational Nurse (LVN) is part of the CHT clinical team, playing a key role in delivering Covenant House’s life-saving intervention to Houston’s homeless young adults, ages 18 to 24.
Specific Responsibilities
- Schedules and completes all health assessments for new (and, if indicated, returning) admissions.
- Schedules physical exams on all new (and, if indicated, returning) admissions.
- Utilization of the current Electronic Health Record system and adhering to policies and procedures pertaining to the system Adhering to HIPAA policies and procedures.
- Patient screening and triage.
- Prepare patients for examinations, tests, and treatment.
- Checks vital signs and records (including height, weight, BMI, temp, BP, pulse, respiration, vision screening, etc)
- Assists medical residents and physicians during physical exams, if needed.
- Phlebotomy.
- Collects samples such as blood, urine, and sputum from patients and performs office tests, as ordered.
- Collect lab specimens, prepare lab requisition, and prepare for transport as ordered.
- 1Calls and/or turn in prescriptions.
- Gives necessary immunizations to residents and staff and records the information.
- Administers medications under the physician’s orders except IV medications.
- Keep both medication carts clean, current, and stocked.
- Review labs and place them in the chart for doctors to review if EHR is down, otherwise review labs in EHR or on the lab website.
- Utilize a tracking system for referral services.
- Answer patient’s questions after consultation with the doctors or as per established procedures (i.e. health questions, medication refills, lab results, etc).
- Fills out medication sheets, and paperwork and creates charts. Ensures that all filing and charts are maintained and up-to-date. Utilizes Athena health or current EHR for record-keeping by scanning important documents into the patient e-charts.
- Maintains all front office services e.g. scheduling outside appointments, and arranging appointments with community services that work with the clinic among other services.
- Maintains record in the ETO or superbill if ETO is down.
- Keep rooms clean and stocked.
- Inventory and requisition supplies and instruments.
- Assist in health education/in-service training for staff when necessary.
- Maintains and rotates medication stock.
- Must complete 24 hours of Covenant House Texas required training per calendar year.
- Performs other duties/responsibilities deemed necessary by supervisor.
- Licensed Vocational Nursing (LVN) degree, with a current Texas license.
- Two years of nursing practice, preferably with adolescents.
- Ambulatory care experience is preferred but not required.
- Must be able to work with distractions, (i.e., telephone calls and interruptions).
- Must be able to rapidly respond/react in emergencies.
- Must have a current CPR certification (infants, adolescents, and adults).
- Must be able to lift up to 5 pounds.
- Must have a valid Texas driver’s license and driving record acceptable to the agency’s insurance carrier.
Salary : $25 - $26
