**Description** **Care Manager RN - Case Management** **The Care Manager RN provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/ or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. Candidates must reside within commutable distance of the following office location: Texas: Lubbock **Required Qualifications:** Upon hire: Associate's Degree in Nursing degree/diploma. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
