The Care Coordinator plays an essential role in the Patient Centered Medical Home approach to health care at Covenant, coordinating and tracking all specialty appointments, outstanding orders, helping patients complete paperwork, communicating with a healthcare team about a patient’s follow up needs and educating patients about resources and options for managing their health.

Job Description:
The Care Coordinator plays an essential role in the Patient Centered Medical Home approach to health care at Covenant, coordinating and tracking all specialty appointments, outstanding orders, helping patients complete paperwork, communicating with a healthcare team about a patient’s follow up needs and educating patients about resources and options for managing their health.
Specific Duties:
- Assist and ensure patients understand the steps they need to take to receive appropriate medical care.
- Provide them with appropriate resources to address barriers to care.
- Consult and collaborate with other health care providers and specialists to help patients schedule appointments when barriers exist.
- Work with patients to identify providers that will accept their insurance.
- Help uninsured patients and patients with limited financial resources find access to needed services.
- Oversee Authorizations for imagining/diagnostics/referrals and work within HER dashboard to facilitate the approvals and needed follow-up.
- Follow up with patients on outstanding orders and referrals to address gaps in care, including yearly health maintenance and screening follow-up.
- Coordinate with clinical resources and providers to ensure smooth continuum of care for
- patients.
- Deal with confidential patient health information including requesting and sending medical records to/from providers with appropriate consents.
- Treat patients with empathy and respect and conduct oneself in a professional manner.
- Communicate effectively and professionally with caregivers, patients, families, and other healthcare providers/facilities.
- Comply with organizational guidelines and health care laws and regulations.

- High School Diploma or equivalent
- 2 years related experience with at least 1 year in a medical office/FQHC experience.
- Excellent verbal and written communication skills.
- Ability to interact positively with peers, executives, patient families and other vendors.
- Demonstrated experience and proficiency with tools, technology and systems typically
- found in a healthcare environment (i.e., Microsoft Office Suite, patient record systems, EMR systems, etc.).
- Degree in Healthcare related studies
- Experience as a Medical Assistant, Certified Nursing Assistant, Pharmacy Tech, OR other clinical or administrative role in a health care setting
- Bi-lingual Spanish Speaking
- Communicate with others in a clear, understandable, and professional manner on the phone and in person, and the demonstrated use of good written and verbal communication skills.
- Ability to positively interact and communicate with peers, executives, patient families and other vendors.
- Excellent written and verbal communication skills.
- Excellent math skills to oversee sliding fee application process.
- Ability to build and lead high-performing teams; must be able to provide clear and accurate direction and guidance.
- Exhibit sound judgment in decision-making.
- Ability to learn and apply new information, knowledge, and experiences in a timely manner.
