Site Manager – Tenant Case Management Program

Position Summary
The Site Manager is responsible for providing non-clinical, person-centered support services to
individuals receiving Tenant Case Management through Covenant Case Management Services.
This position assists members in maintaining housing stability, accessing behavioral health and
community services, building life skills, and supporting whole health and resiliency goals in
accordance with the Vaya Health Tenant Case Management program requirements.

Key Responsibilities
Member Support & Service Delivery
- Maintain scheduled contact with assigned members to support housing stability,
- Assist members with developing and achieving whole health and resiliency goals
- Link members to behavioral health, medical, community, and natural support resources
- Participate in the creation and updates of Person-Centered Plans, Crisis Plans, or
- Provide education, engagement, and support to members and their families/caregivers
- Maintain a working knowledge of the Eviction Prevention Protocol and support
Documentation & Reporting
- Document all service contacts and interventions using required logs and monthly service
- Assist clients with the completion of their annual recertification documentation
- Submit accurate and timely documentation, signatures, and required reports
- Maintain applicant/waitlist tracking as assigned and support Residential Selection
- Maintain confidentiality and comply with HIPAA and Medicaid regulations
- Assist clients with required work order and maintenance forms
- Coordinate services with local vendors
- Work closely with Vaya Health Residential Housing Specialists, ARC of NC Property
- Participate in interviews for housing applicants as requested
- Notify supervisors of member concerns, safety risks, or eviction risks promptly
- Represent the agency in a professional and ethical manner in community settings
Required:
- High School diploma or equivalent
- Valid driver’s license, reliable transportation, and ability to travel to work site
- Ability to pass background check and required credentialing
- Strong communication, documentation, and interpersonal skills
- Experience working with MH/SU/IDD/TBI populations
- Experience in case management, supportive housing, or human services
- Associate or Bachelor’s degree in Human Services, Psychology, Social Work, or related
Work Environment & Physical Requirements
- Field-based position requiring travel to housing sites
- Ability to sit, stand, walk, and lift up to 25 lbs occasionally
- Must maintain professional boundaries and safety practices
