Clinical Coordinator

Job Purpose and Objectives:

Responsible for providing direct nursing care and public health services to individuals, families and the community for Montgomery County.

Position Requirements:

1. High school diploma or other equivalent

2. Bachelor’s degree in Nursing preferred

3. Three years of nursing experience

4. Two years in grant management or public health program coordination preferred

Certifications / Licenses:

1. Registered Nurse in the State of Kansas

Skills/Knowledge/Abilities:

1. Knowledge of public health and community nursing practices

2. Displays excellent grant reporting and compliance, budgeting and project management skills

3. Proficient in Microsoft Office Suite and public health software

4. Excellent time management skills and ability to multi-task and prioritize work

5. Ability to fulfill all duties with minimal supervision and to work independently

6. Assess situations rapidly and make logical decisions in a timely manner

Job Competencies: Essential Functions include the following. • Administers immunizations, medications and treatments as prescribed
• Conducts health assessments, screenings and physical exams
• Provides nursing care in communicable disease control, maternal/child health, family planning and chronic disease management
• Collects and manages lab specimens, interpret results and follows up with patients as needed
• Educates patients and families on health conditions, treatment plans and preventive care
• Offers counseling and referrals for services such as nutrition, mental health, substance use and family support
• Participates in community health programs, health fairs and emergency response efforts
• Provides home visits as part of health promotion activities
• Maintains accurate and confidential patient records using electronic health systems
• Documents services in compliance with departmental, state and federal guidelines
• Reports communicable diseases and other public health concerns as required by law
• Researches available nursing and public health-related grants at the federal, state and private levels
• Collaborates with leadership to assess funding needs and prepare grant applications and proposals
• Develops timelines, budgets and objectives for grant projects
• Monitors grant expenditures and ensures compliance with all reporting and fiscal requirements
• Oversees implementation of grant-funded programs and projects
• Coordinates with internal teams and external partners to achieve grant deliverables
• Ensures project goals align with the county’s public health priorities and strategic plan
• Maintains accurate and up-to-date records of grant activity and performance metrics
• Prepares and submits required reports to funding agencies on time
• Assists with internal audits and evaluations of grant-funded programs
• Serves as liaison between nursing/public health staff and grant funding agencies
• Trains and supports staff on grant-specific procedures and compliance
• Represents the department at local, regional and state-level meetings as needed
• Enforces and maintains compliance with all federal, state and local laws and ordinances
• Performs other duties as assigned or deemed necessary
• Complies with all organizational and departmental policies and procedures
