COUNTY OF GREENE JOB VACANCY

POSITION: County Caseworker 1 – Early Intervention - (Service Coordinator)

(1 Regular Full Time Position)

DEPARTMENT: Human Services-Early Intervention

LOCATION: Fort Jackson Building

SALARY: $18.80/hour

UMWA Bargaining Unit

HOURS: 35 Hours Per Week

8:30 am to 4:30 pm (1 hour unpaid lunch)

BENEFITS: Health, Vision & Dental – First of month following date of hire

OVERALL OBJECTIVE OF JOB

To provide comprehensive case management services for clients served by the Early Intervention Program. Coordinates, monitors and ensures continuity and appropriateness of services. Assists in Early Intervention activities such as data input and LICC coordination, Child Find activities and outreach functions.

ESSENTIAL FUNCTIONS OF THE JOB

(As outlined in Early Intervention, Department of Public Welfare Regulation 4226.52)

  • Coordinating the performance of initial and ongoing evaluations and assessments.
  • Referring at risk children to the tracking system and tracking at risk children.
  • Facilitating and participating in the development, implementation, review, and evaluation of the IFSPs.
  • Assisting the family of an infant or toddler with a disability in gaining access to the early intervention services and other services identified on the IFSP.
  • Facilitating the timely delivery of services.
  • Assisting the family in identifying available service providers and facilitating communication with and between the family and the service provider.
  • Coordinating and monitoring the delivery of the early intervention services.
  • Informing the family of the available advocacy services.
  • Assisting the family in arranging for the infant or toddler with a disability to receive medical and health services, if the services are necessary, and coordinating the provision of early intervention services and other services (such as medical services for other than diagnostic and evaluation purposes) that the infant or toddler needs or is being provided.
  • Offering the family opportunities and support for the infant or toddler with a disability to participate in community activities with other children.
  • Informing the family of community resources.
  • Facilitating the development of a transition plan as part of the IFSP.

OTHER DUTIES OF THE JOB

1. Monitors adherence to regulations; advocates for clients in the Early Intervention Program.

2. Actively participate in LICC functions, trainings, etc.

3. Conducts initial screening to determine eligibility for Early Intervention Program (EIP) including review of medical, clinical and development records and interview of family members and observation of individual.

4. Attend staff meetings.

5. Maintains case management records, charts contacts with clients and updates forms as needed.

6. Submits necessary reports on a timely basis.

7. Enters service notes in Pelican system, as needed, maintaining current billable productivity expectation

8. Maintains all files in an orderly fashion, to be updated monthly.

9. Utilizes computer, software programs and other office equipment, access online resources and or reporting systems as needs to carry out job duties.

10. Participates in Child Find and outreach/education activities as needed.

11. Attends local, regional, and/or State-wide trainings as directed.

12. Performs other duties as required by the Supervisor

SUPERVISION RECEIVED

Receives occasional instruction and supervision from Supervisor in regards to daily work duties.

SUPERVISION GIVEN

None.

WORKING CONDITIONS

1. Works indoors with adequate lighting, ventilation, temperature and workspace.

2. Normal indoor exposure to dust/dirt.

3. Moderate disruptions due to client phone calls.

4. May be subject to stress resulting from client contact.

5. Travels occasionally to providers, clients, meetings or other related county business as required.

PHSYICAL AND MENTAL CONDITIONS

1. Must possess ability to record, convey and present information, explain procedures and follow instructions.

2. Must be able to sit, stand, walk and drive during course of the day for various intervals.

3. Dexterity requirements range from simple to coordinated movements of fingers/hands; feet/legs; torso as necessary to carry out duties of job.

4. Sedentary work with occasional lifting/carrying of objects with maximum weight of ten pounds.

5. Must be able to pay close attention to details and concentrate on work.

6. Must demonstrate emotional stability.

7. Must be able to cope with the physical and mental stress of the position.

QUALIFICATIONS:

A. EDUCATION/TRAINING

(As outlined in Early Intervention, Department of Human Services Regulation 4226.53)

1) 4226.53 (c)(1) A bachelor’s degree from an accredited college or university which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences, and 1 year of full time or full time equivalent experience working with or providing counseling to children, families or individuals with disabilities.

or

2) 4226.53 (c)(2) An associate’s degree, or 60 credit hours, from an accredited college or university in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences, and 3 years of full time or full time equivalent experience working with or providing counseling to children, families or individuals with disabilities.

B. Must possess a valid driver’s license and current FBI, State Police and Child Abuse Clearances.

KNOWLEDGE SKILLS AND ABILITIES REQUIRED

1. Must be able to speak and understand the English language and to follow oral and written instructions.

2. Must possess effective communication skills.

3. Must possess initiative and problem-solving skills.

4. Must possess ability to function independently, have flexibility and personal integrity and ability

to work effectively with clients, co-workers and others.

5. Must possess the ability to maintain confidentiality in regards to consumer information and records.

6. Must possess the technical knowledge of operating personal computers and other office equipment.

7. Must possess a valid Pennsylvania driver’s license and a willingness to travel as needed.

8. Must possess some knowledge of structure, organization and methods in the field of mental disabilities.

9. Must possess some knowledge of principles and methods involved in working socially, physically or mentally disabled adults and children.

10. Must possess some knowledge of federal, state and local policies and regulations in regards to the Early Intervention system and procedures, and the ability to keep updated on all changes affecting the programs.

11. Must possess the ability to skillfully resolve conflict and negotiate effective resolutions.
