Position Summary

Gratiot County’s Commission on Aging is seeking a qualified and service-oriented Caseworker / MMAP Coordinator to assist older adults with complex Medicare and Medicaid matters, nutrition services, and other aging programs. This position plays a key role in overseeing the Medicare Medicaid Assistance Program (MMAP), coordinating Home Delivered Meals services, administering state and local programs, supervising volunteers, and providing education and outreach to the community.

The ideal candidate will demonstrate strong knowledge of Medicare and Medicaid, excellent communication skills, and a compassionate, client-centered approach to serving seniors.

Essential Duties and Responsibilities

Casework & Nutrition Services

  • Coordinate and oversee clients enrolled in the Home Delivered Meals (HDM) program.
  • Conduct required follow-up calls every 60 days and in-home visits every 6 months.
  • Complete comprehensive client assessments with a focus on nutrition and unmet needs.
  • Develop and coordinate service plans using COA services and community resources.

MMAP Coordination

  • Assist Medicare beneficiaries with understanding coverage options and making informed healthcare decisions.
  • Provide guidance and assistance with Medicaid applications and eligibility issues.
  • Supervise, train, and support MMAP volunteers.
  • Deliver public presentations on Medicare, Medicaid, and related topics.

Program Administration

  • Serve as Lead Agency for Senior Project Fresh, including coupon distribution and compliance.
  • Prepare and submit required reports to the State.
  • Assist seniors with Michigan Home Heating and Property Tax Rebate applications and supervise related volunteers.
  • Administer additional state and local programs as assigned, including outreach and education.
  • Maintain accurate documentation of client services, enrollments, and public presentations.

General Responsibilities

  • Maintain confidential and accurate client records using electronic reporting systems.
  • Represent the Commission on Aging professionally in community settings.
  • Perform other related duties as assigned.

Minimum Qualifications

Education

  • Bachelor’s degree in a related field (or equivalent combination of education, training, and experience).

Experience

  • Minimum of two (2) years of related experience.

Licensing

  • Valid Michigan driver’s license and ability to travel for home visits and community presentations.

Knowledge, Skills, and Abilities

  • In-depth knowledge of Medicare and Medicaid programs.
  • Familiarity with community resources and senior services.
  • Strong assessment, service planning, and case management skills.
  • Ability to present complex information clearly to the public.
  • Excellent written and verbal communication skills.
  • Strong organizational, documentation, and data management abilities.
  • Ability to supervise and coordinate volunteers.
  • Demonstrated empathy, professionalism, and advocacy for older adults.
  • Proficiency with computers, Microsoft Office, and CareVantage or similar systems.

Work Environment & Physical Requirements

  • Combination of office work, in-home visits, and community-based activities.
  • Ability to sit and walk for extended periods.
  • Ability to perform repetitive hand and wrist movements.
  • Ability to drive and lift up to 5 pounds.
