Position Objective

Performs difficult work supervising and coordinating program operations for the Aging and Disabilities Division, delegating staff work assignments, training staff on processes and procedures, monitoring operations to ensure compliance with laws and regulations, providing guidance and consultation to employees, monitoring the department budget, completing required reports, and related work. Work is performed under the general direction of the Aging and Disabilities Division Manager.

To supervise the Unit to assure that quality services are provided to Chisago County Residents

  • To monitor and evaluate employee MnCHOICES assessments/caseloads to ensure that services are offered and provided on a timely basis and to meet client needs
  • Corrective action plans will be completed for cases in non-compliance
  • Case openings, closings and requests for information will be reviewed for agency standards compliance
  • Case consultation will be available minimally twice monthly and as needed to case managers
  • Monitor the completion of time studies and case reviews
  • Ensure the compliance of federal, state and county rules and regulations
  • New or updated in rules and statutes and policies will be discussed and disseminated, with accompanying staff orientation.
  • Printed information will be provided to staff in the form of bulletin and agency procedures
  • Evaluate ongoing community needs, quality of services and appropriateness of programs on a continual basis in the areas of Long-Term Services and Supports (LTSS) to Elderly and disabled residents through HCBS waiver programs (AC, DD, CADI, CAC and BI) and CFSS, and delivery of MnCHOICES.
  • Report to the Division Manager, Department Director and/or the County Board on information pertinent to the population, and other Department interests as assigned.

To recruit, develop and manage the Unit staff to ensure that quality professional practices are delivered.

Carries out management of personnel reporting to the position, following state, federal, county, and bargaining unit labor laws.

  1. Requisitions, interviews, contributes to the selection of persons desirable to the mission and role of the county and the Department.
  2. Conducts annual performance review, recommends, develops and implements corrective action plans, or follows steps for termination, those personnel under his/her purview as identified through the practice of supervision.
  3. Develops training plan for positions under supervision to the position.
  4. Assists the Department in management of personnel in other programs of the Department as assigned.

To orient, develop and train staff

  1. An orientation plan including policy manual will be reviewed with the employee weekly during the first two months; twice monthly during the third and fourth months; a written evaluation in the fifth month prior to the end of probation at six months
  2. A mutual assessment of training needs for each assigned employee and review of position description will be completed at the employee’s annual evaluation
  3. Promote staff development via group/individual training

Provide supervision to the individual staff and hold primary responsibility for the overall assignments and functions of the unit, reporting status and problem solving if relevant with Division Manager, and/or using other Department resources if needed, i.e. mental health consultation, county attorney consultation, etc.

Ensures fiscal responsibility of the services managed by the program assigned.

  1. Adult Services Budget
  2. MnCHOICES State and Federal Funding Administration
  3. Conduct fiscal host responsibilities as assigned
  4. Medicaid Waiver Aggregate management

Funding systems used by staff will reflect maximum use of state and federal resources to conserve county money

Work with Division Manager and Department Director to keep informed, provide requested data and find solutions for funding problems in order to ensure adequate funding

  1. Provide Division Manager with data and reports that indicate activities that reflect on budget planning
  2. Provide program data requested in an accurate, timely manner; identify and analyze trends that impact funding streams to the Department.
  3. Ensures that program / client data on work floor is accurately reflected in the social services information system.

To cooperate with the Director in areas relating to the County Board of Commissioners.

  • As assigned, present special reports to the County Board with accurate and appropriate information
  • Administrative items for agendas will be turned in to the Division Manager one week before Board.
  • Staff members will be encouraged to submit reports as directed by the Board, Director, and Division Manager.
  • Adequate preparation will be made to respond appropriately and accurately to questions and concerns at board meetings

Develop and present Board recommendations regarding policy, programs, grants, etc. in a professional manner

  • Adequate research will be completed prior to recommendations so that basis for action is clearly established
  • Periodic reports on programs, trends, unit operation, etc. will be shared routinely to keep the Board aware and knowledgeable as requested by the Director and Board
  • New recommendations or data will be discussed with the Division Manager and Director prior to submitting to Board.
  • Maintains and develops good working relationships with internal and external partners. Participate in staff meetings and conduct unit meetings at least 12 times a year with printed agendas
  • Will coordinate unit activities and personal work assignments within the Department and with other Chisago County Departmental partners.
  • As changes occur in programs that interact with other areas of work in the Department or County, meetings will be scheduled to discuss effects on service delivery and other partners.
  • Approaches new information in a problem-solving way within the Department and with partners of the Department.
  • Maintain contacts with Chisago community resources to identify and work toward solutions of any problems that arise
  • Provide data regarding service needs to local providers
  • Attend/monitor or delegate attendance at special committees and boards as assigned that are of interest to the Department.
  • To assess need for changes in reporting requirements due to SSIS and identify additional data needs minimally on a yearly basis
  • To assess SSIS reports and determine what data can be used for agency statistical reports on a yearly basis.

Manages and assures compliant delivery of Medicaid waivers.

  • Manages the Chisago County Medicaid aggregates within the parameters established by the County and State of Minnesota.
  • To monitor allocated waivered funds and report the status to the Division Manager monthly
  • To maximize the usage of waivered funding, maintain close contact with DHS and individuals with expertise in the waivered area
  • As needed: Manage and supervise Vulnerable Adult Protection Assessments and ongoing adult protection plans.
  • Assures delivery of adult protection services within state statutes, local guidelines and best practices for this body of work.
  • Works as a team with other professionals within the Chisago County Department of Human Services in the delivery Adult Protective Services.
  • Manage and supervise services to the elderly and disabled populations and application(s) of MN Choices.
  • To assure compliance with LTCC/AC program and funding guidelines
  • Maintain and distribute current data to employees
  • Ensure the implementation of the state requirements
  • Provide leadership and recommendations to the Division Manager on a regular basis regarding the implementation and maintenance of the MN Choices/ LTCC program.
  • Maintain MN Choices certification.
  • Monitor and supervise to assure county optimal performance relating to third party contracts for services to consumers.
  • Ensure performance of county relating to third party contracts for services rendered by county.
  • Take corrective action in obtaining additional funds allocations as needed
  • Works with Department contracting personnel in pursuit or new or revised contracts, or in the procurement and delivery of contracted services.
  • Coordinates activities with Chisago County Public Health Division and staff within.

Demonstrates adherence to and knowledge of HIPAA and Data Practices in work assignment.

Desired Minimum Qualifications

 

Bachelor's degree in Social Work, Psychology, Sociology or related field AND Four years profession work in a Social Services or Public Health agency

OR

Bacheler’s degree in Nursing, Public Health or related field AND Four years professional nursing, or related experience, in provision of public health services to individuals and families at the community level

Required license(s):

Valid driver’s license

Licensure and registration, if applicable, as Registered Nurse and Public Health Nurse in Minnesota
