Chester County MH/IDD seeks an Intellectual Disability Supports Coordinator to facilitate, coordinate and monitor services for individuals with intellectual disabilities. Supports Coordinators work with consumers and families in their homes and/or in provider agencies. Job responsibilities include frequent scheduling and facilitation of meetings and entering documentation into a statewide database. The incumbent should have excellent organizational, communication and computer skills. This position requires travel to attend meetings off-site.

Current Child Abuse, State Police and FBI clearances are required, as is a writing skills assessment and an online assessment.

Candidate must have a bachelor's degree which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science;
or
Two years experience as a County Social Service Aide 3 and two years of college level course work, which includes at least 12 college credits in sociology, social welfare, psychology, gerontology and criminal justice or other related social service;
or
Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Serivces Aide 3 or similar position performing paraprofessional case management functions.

Essential Duties, Tasks and Accountabilities:
The position provides case management services to a group of clients with intellectual disabilities living in various community living arrangement programs or their own homes throughout the county.  This includes initial intake interviews, appropriate referrals, liability information and completion of progress notes and client contacts.
  • Assess client needs and develop proposed services plans/life management plans on an annual basis.
  • Ensure implementation and coordination of the plan.
  • Monitor client programs as per established case management standards.
  • Maintain records, correspondence, service plans and authorizations for each client.
  • Make necessary referrals and coordinate services as prescribed in the client's proposed services plan/life management plan (e.g., core team, in-home therapy, camp, residential services, day program services, etc.).
  • Have weekly supervisor conferences on specific cases of caseload.
  • Prepare monthly case management statistics.
  • Attend staff conferences of other agencies as they relate to case management services.
  • Ensure a close working relationship with other human services agencies as they relate to the assigned caseload.
  • Close cases no longer necessitating support coordination services.
  • Perform other duties, as assigned by Supervisor.
Minimum Requirements:
The ID Support Coordinator (County Caseworker) position comes under the provision of the Child Protective Services Law.  If you are a final candidate for this position, you will have to provide reports on your background from such sources as the Pennsylvania State Police, Federal Criminal History, and the PA Child Abuse History Clearance.  If your background is unacceptable, you will be disqualified for employment in this position.  You will receive information about these requirements and how to obtain the required reports at the time you are being considered for employment.
 
  • A Bachelor's degree from an accredited college or university with a social welfare or social work major; or
  • An Bachelor's degree from an accredited college or university which is supplemented by 12 college-level credits from an accredited college or university in sociology, social welfare, psychology, gerontology, criminal justice or other related social sciences and one year of professional social casework experience in a public or private services agency; or
  • An equivalent combination of experience and training which includes 12 college-level credits from an accredited college or university in sociology, social welfare, psychology, gerontology, criminal justice or other related social sciences.
  • Must have a valid driver's license and use of a private vehicle.
     
    Clarification of Requirements: You may substitute appropriate experience for the required education, except for the 12 college-level credits from an accredited college or university in specified areas, or appropriate education for the experience.  Unrelated experience or education will not be accepted.
     
If you do not have a Bachelor's degree from an accredited college or university with a major in sociology, social work, social welfare, psychology, gerontology, criminal justice or other related social sciences, you must submit a list of college courses or college transcripts showing the completed 12 college-level credit hours from an accredited college or university required in these areas.
 
Preferred Skills, Knowledge & Experience:
  • Bachelor's degree from an accredited college or university in Social Science or related field.
  • Two years of experience in the human services field.
  • Experience with individuals who have developmental disabilities.
  • Experience and knowledge in child development.
  • Background and knowledge in social work and psychology.
  • Minimum of one year of experience in the social work field.
  • Basic medical and psychiatric knowledge.
  • Proficiency in mediation.
  • Knowledge of Part C of IDEA and early intervention regulation.
  • Knowledge of Mental Retardation Bulletins.
  • Knowledge of MH/MR Act 66.
  • Working knowledge of the SS/DPW system.
Relevant background checks, clearances, physicals, drug tests, and any other assessments or evaluations will be conducted after an offer of employment is given per County policy.

To perform this job successfully, an individual should have:
  • Basic Word skills
  • Basic OutLook skills (Email and Calendar)
  • Basic Internet skills (for research purposes)
  • Ability to learn/utilize computer systems for everyday work.
