Speech Language Pathologist (SLP)
We are seeking a dedicated and compassionate Speech Language Pathologist (SLP) to join our interdisciplinary team at Countryside Post-Acute and Rehabilitation Center in Lawrenceburg, Tennessee. As an SLP, you will play a vital role in helping our patients regain their communication skills and achieve their rehabilitation goals.
Speech Language Pathologist (SLP) Responsibilities:
Evaluate and diagnose communication and swallowing disorders in patients with a range of conditions and diagnoses
Develop and implement individualized treatment plans to address patient communication and swallowing needs
Provide speech therapy services to patients, including individual and group sessions, as well as carry-over exercises and activities
Collaborate with other healthcare professionals to ensure comprehensive care and goal achievement
Document patient progress and treatment plans in accordance with facility policies and procedures
Participate in patient care conferences and family meetings to discuss treatment progress and goals

Clinical fellowship (CF) complete

Excellent communication and interpersonal skills
