Country View of Sunbury, a skilled nursing facility located in Sunbury, OH, is seeking a dedicated Full Time, experienced, MDS Nurse (RN). The successful candidate will be responsible for the following duties:
JOB SUMMARY:
The purpose of this position is to assess residents’ physical and mental function and document
data on minimum data set forms completely and accurately. The MDS Coordinator-RN is also
responsible for the documentation of all additional assessments required completely and
accurately. The MDS Coordinator-RN shall determine appropriate referrals to other health care
professionals and use the Care Area Assessment protocols to determine whether to proceed or not
proceed with the overall care plan
RESPONSIBILITIES AND MAJOR DUTIES:
1. Interview residents, residents’ representatives and others with information related to the
minimum data set assessment.
2. Examine residents to collect information related to the minimum data set assessment.
3. Review medical records and records related to residents’ health status to collect information
related to the minimum data set assessment.
4. Enter data into the computer program for minimum data set assessments.
5. Print minimum data set assessments are related reports from the computer program and take
follow-up action as necessary.
6. Review reports of timely completion of residents’ Care Area Assessments and care plans and
take follow-up action as necessary.
7. Participate in the Quality Assessment and Assurance Program, for the nursing department, in
cooperation with the Quality Assurance Coordinator.
8. Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.
9. Complete reports and assignments accurately and adhere to established time schedules.
10.Establish open lines of communication between all nursing personnel.
11.Consistently work cooperatively with administration, nursing service personnel, physicians,
community agencies, residents, families and consultants.
12.Act as a licensed nurse when requested and provide care and treatment to residents as
requested.
13.Observe infection control procedures.
14.Follow Residents’ Rights policies at all times.
15.Adhere to all facility policies & procedures and ensure they are adhered to by all responsible
parties and carry out disciplinary action for non-compliance as appropriate.
16.Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all
times.
17.Perform other related duties as directed by the Administrator and/or person of authority.

POSITION QUALIFICATIONS:
EDUCATION AND EXPERIENCE
Graduate of an accredited Registered Nurse program. Bachelor’s Degree preferred. Previous
experience as a MDS Coordinator preferred. 1 year of experience as a RN with direct experience in
a Skilled Nursing Facility required. Ability to add and subtract and perform basic mathematics as it
relates to the position is required. Ability to read/write/comprehend the English language in written &
verbal form required. Ability to communicate both verbally and non-verbally required.
LICENSURE, REGISTRATION, COMPLIANCE CHECKS AND/OR CERTIFICATION
Successfully complete &/or comply with all required background checks to include but not limited to:
Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG
list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List.
Valid License to practice as a RN in the State of Ohio. CPR certification required.
RESPONSIBILITY FOR PATIENT CARE
Able to review and interpret clinical information related to the role of the MDS Coordinator and RN in
a Skilled Nursing Facility. Provide services based on physician orders. Communicate physician,
patient and family needs with the appropriate caregivers.
COMPUTER
Intermediate personal computer skills including electronic mail, routine database activity and word
processing.
LEADERSHIP RESPONSIBILITIES
Responsible for the direction and management of any/all MDS Coordinator – LPN’s within the facility.
RESPONSIBILITY FOR CONFIDENTIAL MATTERS
Works regularly with information such as medical records or reports, financial reports, information on
computer screens or other electronic devices, and records that are restricted to specific persons.
Must comply with confidentiality policy.
