Country Manor's Health Care & Rehab Center is now hiring for a full-time Licensed Social Worker! This role directly impacts the quality of life for our residents and patients!

We are in search of an LSW to serve a designated number of residents, families and guardians on our Rapid Recovery (Short Term Care) unit. As an LSW, you will assist us in maintaining our distinctive level of care by collaborating with residents/patients and their families through admission to discharge, developing a plan of care, monitoring progress and ensuring success in accordance with the procedures of Country Manor and the Social Services Department.

The ideal candidate will:

Responsibilities include:

• Collaborate with the multidisciplinary health care team
• Interview residents/patients/representatives and assess their current condition, needs, strengths, and weaknesses
• Adhere to and follow the Social Work Code of Ethics and Standards of Practice
• Develop and implement resident/patient-centered plans of care
• Monitor and evaluate clients’ progress and modify treatment plans accordingly
• Maintain accurate records and report on clients’ status
• Refer clients to community services and assist with coordination of needs to ensure successful discharge plans

Qualifications:

• Must hold a current MN license or License in Process from the MN Board of Social Work
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse population and groups
• Resilience, along with the ability to assess situations and problem-solve for therapeutic outcomes

This is full-time Monday thru Friday position. Occasional weekend/holiday/on-call.
