Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our dynamic healthcare team. In this vital role, you will provide skilled nursing care to patients in their homes, supporting recovery, chronic disease management, safety, comfort, and independence. This role includes assessing patient needs, developing and implementing individualized care plans, administering treatments, educating patients and families, coordinating with the interdisciplinary care team, and documenting care accurately and timely.
Key Responsibilities
· Conduct initial and ongoing patient assessments, including vital signs, medication review, pain level, functional status, safety risks, and changes in condition.
· Develop, implement, and update individualized nursing care plans in collaboration with physicians, therapists, patients, families, and other members of the care team.
· Monitor patient progress, identify changes in health status, and promptly report concerns to the physician or appropriate clinical supervisor.
· Educate patients and caregivers on disease management, medication use, wound care, fall prevention, infection control, nutrition, home safety, and when to seek medical attention.
· Coordinate care with physicians, physical therapists, occupational therapists, social workers, home health aides, pharmacies, and community resources.
· Complete accurate, thorough, and timely clinical documentation in accordance with regulatory requirements, payer standards, and agency policies.
· Support patient independence by teaching self-care skills and recommending appropriate tools, equipment, or services to improve quality of life.
· Follow infection prevention, patient privacy, emergency preparedness, and safety protocols during all home visits.
· Participate in case conferences, quality improvement activities, and care planning meetings as assigned.
Required Qualifications
· Current, unrestricted Registered Nurse license in Ohio.
· Graduate of an accredited nursing program; Associate Degree in Nursing required, Bachelor of Science in Nursing preferred.
· Current CPR/BLS certification.

Pay: $28.00 - $30.00 per hour
