Key Responsibilities:
Lead and oversee social services programming to address the emotional, psychosocial, and long-term care needs of our residents.
Advocate for resident rights and dignity while supporting their autonomy and well-being.
Guide residents and families through community resources, financial support options, and discharge planning.
Mentor and support the social services team to deliver compassionate, person-centered care.
Ensure compliance with federal, state, and local regulations through effective policy and procedure management.
Coordinate resident admissions, discharges, and transfers to ensure smooth transitions.
Maintain clear, accurate, and timely documentation in accordance with regulatory standards.
Qualifications:
Bachelor’s degree in Social Work, Psychology, Sociology, or a related field - preferred.
Licensed Social Worker (LSW) in Iowa.
Experience in social services, especially in long-term care, is a plus.
Strong leadership, communication, and problem-solving skills.
