Case Manager

Country Court Skilled Nursing & Rehabilitation Center is a locally and family-owned nursing facility in Mount Vernon, Ohio. We are looking to add a Case Manager to our team. We are looking for an individual that is a strong team player, has good communication skills, is caring and compassionate, and has a desire to make a difference in the lives of our residents. Previous experience as a Case Manager in long-term care is a positive but is not a requirement. Job Description and Duties will include, but are not limited to;

• Coordinate care plans in collaboration with nursing, therapy, dietary, and physician teams
• Ensure resident needs, preferences, and psychosocial concerns are addressed in care planning
• Monitor changes in resident condition and communicate updates to appropriate team members
• Complete Admission paperwork with new admissions
• Coordinate safe and timely discharges, including:
• Home health services
• Durable medical equipment (DME)
• Transportation arrangements
• Provide discharge education to residents and families
• Serve as a primary point of contact for residents and families regarding concerns
• Assist with advance directives and code status discussions (in coordination with nursing/physician)
• Contribute to MDS assessments (especially Section Q and psychosocial areas)
• BIMS
• PHQ9

Pay: From $22.00 per hour
