Substance Use Disorder/Addiction Outpatient Counselor

Substance Use Disorder/Addiction Outpatient Counselor Position(s) with a CARF Accredited and Licensed Organization

· Full-time (40) hours a week, with (9) hours covering early evening (4-7pm or 5-8pm groups) or can apply for Part-time (3 days a week, 11:00am-8pm)

Qualifications:

· Prefer a master’s degree in health care field with credentials completed (LPC, LMSW, LCSW, or LAC).

· Applicants can apply if you have PLPC/CSW/MSW/CIT or in process of completing licensing, with a BS/BA degree, minimum.

· Has 2 years work experience with treatment of substance use disorders (SUD)

General Essential Duties:

· Conducts screening/assessments with clients for eligibility and appropriate level of care.

· Complete required record & report documentation for client’s files in electronic health records.

· Carries caseload up to 12 clients within the scope of practice based on credentials. Collaborates with interdisciplinary team, and referrals, during treatment and discharge planning process.

· Conducts group sessions, individual sessions, educational meetings and co-facilitates with Team, when able.

· Formulates primary treatment plan(s) with the client’s input.

· Utilizes the most current and best practiced counseling methods and techniques.

· Monitor, evaluate, and record client progress in accordance with the treatment plan.

· Assist with the utilization review process with admin staff by providing complete and accurate client information to billing and/or the health insurance provider when requested.

· Recognize, de-escalate and address clients in crisis, where needed.

· Conduct discharge planning and refer clients and/or families to appropriate community resources.

· Conduct collection of and complete forms for client drug testing on site.

· Adhere to any applicable laws, regulations, and agency policies governing HIPAA compliant substance use disorder treatment.

Work Location: In person
