The Reconnect Program is designed to identify and reduce mental health symptoms in adults age 60 and older in Orange County. It integrates evidence-based practices into culturally diversified care management and life coaching services, consisting of comprehensive psychosocial assessments, mental health screening, and supportive counseling, linkages to resources, support groups, and behavioral activation. 


Position Objective: Orange County is home to the fourth-largest population of veterans in California with 130,000 veterans. 60% of them (78,000) are older adults. This number is greater when including military-connected family members. The objective of this position is to improve the quality of life and overall wellbeing of Orange County older adult veterans aged 60 and older by providing culturally responsive case management services, care coordination, and assisting them in overcoming existing barriers to thrive later in life.


Job Functions (including but not limited to):

· Conduct comprehensive in-home screenings and assessments.

· Develop and implement individualized care plan to address unmet needs.

· Assist older adult veteran and their military-connected families in accessing appropriate VA benefits and appropriate community resources.

· Provide care navigation, coordination, and education.

· Conduct veteran focused group activities.

· Conduct community outreach and collaborate with veteran service organizations (VSOs) 

· Offer leadership and supervision to assigned support volunteers.

· Compile and report case load statistics in a concise, timely manner to produce a monthly report.

· Other duties as assigned by supervisor.


 Reporting To:  Senior Social Worker

Requirements

Qualifications:

· BA in social work, health science, gerontology, human services, counseling, psychology, communication, or related fields, and/or relevant life experience in these areas.

· Must be a Veteran or a military-connected family member. 

· Knowledge of Orange County community services and resources preferred.

· Ability to work effectively with diverse populations and in team settings.

· Outstanding communication skills.

· Excellent analytical, problem solving and organizational skills. 

· Ability to develop a sense of trust and rapport with program’s clients.

· Ability to work independently and multi-task in achieving desired goals managing time efficiently in a fast pace environment. 

· Ability to maintain objectivity and confidentiality in documenting and discussing caseload.

· Proficient in modern office practices including basic computer skills.

· Proof of eligibility to work permanently in the United States.

· A valid California driver’s license, proof of current vehicle insurance, reliable transportation.

· Submit and successfully complete a LiveScan fingerprinting background check.

While performing the duties of this job the employee will be required to sit for periods of time. Standing, walking and bending are required movements. The employee might need to lift approximately 25 lbs.


Conditions of Employment:

The ability to transport oneself to needed appointments with reliable transportation is required.
