JOB REQUIREMENTS
A. EDUCATION: Four-year degree in social work or related field such as psychology, sociology, nursing, gerontology or related field.
B. EXPERIENCE: Two years’ experience in related field. Additional experience may substitute for education.
C. SKILLS: Thorough knowledge of theories, principles and practices of social service programs; knowledge of problems affecting the social, psychological and economic well-being of the elderly; ability to visit clients in-home without physical assistance; effective communication, both verbally and in writing.
NATURE OF WORK
This position works directly with older adults on a daily basis and is responsible for the planning, arrangement, and coordination of appropriate community-based services for eligible clients. The Case Manager collaborates with and provides support to the Senior Programs Director as needed. The role requires maintaining all program-specific training and certifications, as well as demonstrating strong verbal and written communication skills.
POSITION RESPONSIBILITIES
1. Screen client and perform a comprehensive client assessment upon receipt of a referral form or direct contact. Evaluate and address the client’s continuing changes and needs.
2. Determine client eligibility under existing rules and established regulations.
3. Identify nature and extent of services needed and make referrals to assist the client in obtaining those services.
4. Develop care plans in conjunction with the client, the client's family or physician, or other involved persons, evaluating the client's need and goals to be achieved. Anticipated service outcome and people and agencies involved and responsible for service provision.
5. Review and supervise implementation of care plan. As well as develop informal support system to maintain the client in the community.
6. Provide follow-up on service arrangements and referrals within two weeks.
7. Arrange care for caregivers and counsel with caregivers regarding stresses of caregiver role.
8. Arrange for training of caregivers, family members or friends as needed. As well as complete written client reassessment annually and more frequently if necessary.
9. Case Managers shall monitor all services to ensure the services effectively address the issues that prompted the need for intervention and produce positive outcomes.
10. An understanding of social service practices, aging issues, caregiver support, and
community resources.
11. Complete and maintain client record with progress reports and forms related to service provision and monitoring.
12. Ability to make independent home visits and conduct assessments face-to-face.
13. Maintain valid driver license, auto insurance as well as a reliable transportation in order to make client visits.
14. Maintain accurate, complete, and timely client records in accordance with DOEA, AAA, and agency policies.
15. Document all client contacts, home visits, travel time, communications, service monitoring, and case notes.
16. Maintain accurate, complete, and timely client records in accordance with the agency's policy.
17. The Case Manager works in conjunction with Adult Protective Services to ensure that high-risk clients receive timely and appropriate crisis-intervention and resolution services.
18. In the event the President of the United States or the Governor of the State of Florida declares a disaster or a state of emergency, the Case Manager shall perform his/her duties under the direction of the President / CEO as prescribed by the COASL Disaster Preparedness Plan in order to implement emergency relief measures and/or activities. All actions under this plan shall be for the purpose of ensuring the health, safety and welfare of the elderly in the disaster area.
19. Display initiative in presenting supervisors with new ideas to benefit the Council, its staff and its clients.
20. Maintain a courteous and professional demeanor at all times with supervisors, co-workers, clients and the general public.
21. Other duties as required by the Senior Programs Director.
