What are the responsibilities and job description for the Case Manager position at Council on Aging, Inc?
Area includes Mercer County Job responsibilities for this position include but not limited to; Completing case management in-home assessments for the Aged and Disabled Waiver program . Other responsibilities include assisting members obtain their needs to remain living in the community. Prior work experience in this field is preferred but not required. Required skills include communication, organization, and ipad knowledge. Required education includes a 4 year degree in a human services field or a social work license. Valid drivers license and reliable transportation a must. This position reports to our Princeton office and then goes out into the field to do home visits. Applicant must also pass a WV Cares background check. All Care Home and Community Services, Inc. is an EOP and EOE.
