Coulson Capital is launching Wellness Care, a new care coordination platform focused on Chronic Care Management (CCM) and Transitional Care Management (TCM). Our goal is simple and ambitious: keep high-risk patients from “falling through the cracks” after hospital visits, and wrap them in ongoing support between appointments. We partner with physicians, health systems, and payers to deliver proactive, preventative care—starting in Texas and scaling nationally.

We’re building a data-driven, high-touch enrollment engine to rapidly grow our CCM program in multiple states, and we need a Director of Enrollment to develop and lead the team of Enrollment Specialists who are key to that growth. '


Role Summary

The Director of Enrollment is responsible for building and leading Wellness Care’s patient enrollment engine, with a primary focus on rapid, compliant patient acquisition into CCM, TCM, and related care coordination programs.

This leader will design, staff, and manage a high-performing Enrollment Specialist team (scaling to 200 Enrollment Specialists over 2026) that converts eligible patients from HIE feeds, TCM discharges, referrals, and inbound interest into active, consented participants.

You own the top of the funnel for patient growth and are accountable for enrollment strategy, execution, and performance.


Key Responsibilities

  • Own the patient enrollment and acquisition strategy for CCM, TCM, and related programs, hitting aggressive monthly and annual growth targets.
  • Build, lead, and scale a high-performing Enrollment team to 200 Enrollment Specialists in 2026, including supervisors, QA, and training functions.
  • Design and continuously optimize outbound and inbound enrollment playbooks (HIE/ADT feeds, TCM discharge lists, provider referrals, and inbound interest).
  • Run the day-to-day operations of the enrollment contact center: staffing plans, scheduling, performance huddles, QA, and workflow optimization.
  • Partner closely with Clinical Operations, Scriptful physicians, Marketing, and Revenue Cycle to ensure seamless handoffs, compliant documentation, and strong TCM → CCM conversion.
  • Ensure all enrollment activity complies with Medicare, payer, and HIPAA standards, with clear patient consent and excellent patient experience.
  • Use data and dashboards to manage core KPIs (contact rate, conversion rate, time-to-enrollment, cost per acquisition, quality scores) and drive continuous improvement.


Qualifications

Education

  • Bachelor of Business Administration, Healthcare Administration, Operations Management, or related field required.
  • Master’s degree (MBA, MHA, MPH) preferred but not required.

Experience

  • 7–10 years of experience in high-volume sales, enrollment, or contact center leadership in healthcare or a similar regulated environment.
  • Proven track record building and scaling teams to 100 front-line staff; scaling to 200 experience is a strong plus.
  • Experience with Medicare programs (CCM, TCM, population health, ACOs, FQHCs, or health plans) strongly preferred.
  • Demonstrated success in rapid patient/member acquisition, contact center optimization, or outbound campaigns.

Success Metrics

  • Achievement of monthly and annual enrollment targets (by program, state, payer).
  • Conversion rates from eligible to enrolled patients, especially TCM → CCM.
  • Speed from referral/ADT alert to completed enrollment.
  • Enrollment team productivity (calls, contacts, enrollments per FTE) and quality scores.
  • Retention and engagement of Enrollment Specialists.
  • Compliance audit outcomes and patient satisfaction related to the enrollment experience.
