Job Summary
As an extension of the clinical and social work team, the Unit Admission & Discharge Coordinator supports the daily operations of the Ray of Hope unit. This role manages administrative and communication tasks, coordinates admissions and discharges, and collaborates with multiple departments and outside facilities. While primarily non-clinical, the Coordinator may also provide patient sitting or basic clinical assistance when needed.
Duties and Responsibilities

Greet, direct, and support patients, visitors, and hospital personnel.

Assemble and maintain accurate patient charts and ensure timely documentation.

Receive and disseminate telephone messages appropriately.

Support admissions by coordinating with patients, guardians, facilities, and providers.

Complete insurance benefit checks as needed and request outside records.

Assist with discharges by faxing records, arranging transportation, and transmitting documentation to PCPs or accepting facilities.

Track and communicate referral status; follow up on declined referrals.

Monitor patient charge items and support scanning processes.

Act as the communications hub during emergency situations.

Provide clerical support such as meeting minutes and supply ordering.

Sit 1:1 with patients or provide basic clinical support if needed.

Collaborate with the multidisciplinary team to ensure efficient patient flow and transitions of care.
