Job Description

Corvallis Housing First is a growing organization dedicated to providing support and housing to individuals experiencing homelessness. We currently operate four supportive housing facilities, conduct outreach housing case management in partnership with local shelters and outreach programs, as well as provide support for people in community-based housing. Following the philosophy of Housing First, we work to provide low-barrier, behavior-based housing without preconditions and barriers to entry, such as sobriety or treatment participation requirements.

The Lead Case Manager’sposition is primarily responsible for work in three areas, providing case management to small caseload of clients, supervising a 2-5 member team, and working with other partners in the community to collaborate on care. As a case manager, they ensure that their clients’ needs are assessed, collaboratively sets goals with the client, and connect them to community services such mental and physical health care, housing, job training, and financial support programs. The Lead Case manager ensures their team is providing effective services for clients using and coaches team members in how to do their work, following CHF policies and procedures. The Lead Case Manager is charged with setting the tone for our client services, following trauma-informed care, harm-reduction, and other best practices in the field, guiding staff on practical day-to-day issues with clients, consulting with supervising staff when appropriate, and assisting with program reporting.

We are looking for someone with experience working successfully with other staff to achieve positive outcomes for clients; who is organized, timely, and kind, but who can also provide structure and model professional judgement, behavior, and healthy boundaries. We are passionate about what we do and the work we do as a team to help people who have traumatic backgrounds live their best life. Being practical, self-reflective, non-judgmental, calm in stressful situations, and possessing a sense of humor are necessary.

Specific duties:

Work with Clients

  • Build rapport and trust with clients, practicing non-judgment and motivational interviewing skills to better understand their individual needs and goals. Practice empathetic listening and communication.
  • Teach and model positive interactions to build a greater sense of self and to help clients move forward in wellness and recovery.
  • Set individual service plans with clients who chose to participate, utilizing strengths-based case management while embracing the philosophy of Housing First. Utilize assertive engagement techniques to meet clients where they are and coach clients in setting goals and identifying realistic steps in order to achieve them.
  • Maintain accurate and up-to-date documentation associated with assessment, client interaction, coordinated services, residents' progress, and other activities, as required.
  • Assist participants through the housing process, including assessing housing needs and barriers, identifying possible housing options, locating apartments, assisting with the application process and financing, and connecting clients to community resources. Work with case workers and other staff on move-in planning and support.
  • Complete grant-specific processes and paperwork as required. Complete data entry into Shelterware, and HMIS (Homeless Management Information System) as required.
  • Collaborate with other providers involved in client care. Initiate communication regarding shared clients in order to support clients in meeting their needs. Ensure follow-through on referral process and be inclusive of other service providers in case management process. Advocate for each individual when there are difficulties obtaining needed services in the community.
  • Assist clients in developing and accessing natural community supports as well as community services. Promote active and appropriate inclusion of family/identified individuals in treatment and support planning.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Transport clients as needed.

Program Development and Collaborating with Partners

  • Work with the Program Director to assist in developing and revising program policies and procedures, evaluation processes, exploring new approaches to client services, while assessing potential impacts to systemic inequity.
  • Assist and supervise the documentation process required for billable services.

Staff Supervision

  • Ensure best practices are integrated into client services and housing operations including Harm Reduction, Trauma Informed Care, Motivational Interviewing, cultural competency, and assertive engagement.
  • Ensure a positive, consistent approach to our work with clients.
  • Ensure that organizational policies and procedures are being followed consistently.
  • Consistently use and model strong professional judgment while providing services to clients, provide consultation to other case manager/case worker staff on troubleshooting practical issues, recognize instances where consulting with supervising staff or community partners is necessary/appropriate.
  • Help ensure that CHF services are continuously evaluated and improved upon when necessary, to best serve clients. Monitor data entry, ensuring accuracy and that it is completed in a timely manner. Work with the Program Director to complete grant reporting responsibilities as needed.
  • Participate in collaborative partnership meetings and processes as indicated for specific program partnerships.
  • Responsible for onboarding, scheduling, and training new staff, as well as ongoing supervision and evaluation of staff. Ensure one-on-one check-ins with team staff on a regular schedule, to be determined by staff needs.
  • Model appropriate self-care practices and advocate for staff self-care to avoid burnout.
  • Be on call after hours approximately one weekend a month (Friday evening-Sunday evening). On-call shifts at CHF are generally uneventful, incidents occur generally once or twice per month.

Other responsibilities include:

  • Participate in weekly staff meetings, CHF housing meetings, and relevant trainings as required.
  • Drive personal car to meet with clients in their homes and in the community. Must possess a current driver’s license, maintain vehicle insurance coverage. Occasionally transport client to appointments, etc. if necessary.
  • Carry a CHF provided cell phone and laptop computer in order to complete work in the field.
  • Practice confidentiality in all dealings with clients and staff.

Required Experience/Abilities:

  • Must have a bachelor's degree in a social service area (Psychology, Human Development and Family Science, Psychology, Social Work, Public or Community Health, etc.) OR equivalent training/experience
  • At least 3 years of case management or related experience in serving people experiencing homelessness.
  • At least 2 years of experience supervising other staff
  • Ability to pass a background check.

Preferred Experience/Abilities:

  • Certified Community Health Worker or Peer Support/Wellness Specialist

Pay: $23.00 - $25.00 per hour
