Job Overview
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will play a vital role in supporting individuals and families through various challenges, including behavioral health issues, addiction recovery, child welfare, and housing stability. The Case Manager will work closely with clients to develop tailored plans that promote stability, recovery, and overall well-being.
Duties
- Hours for this position may vary but are typically Monday-Friday.
- The case manager for the RISE program is responsible for release services for detainees incarcerated at the Lincoln County Detention Center (LCDC). LCDC is an adult correctional facility of unadjudicated individuals.
- The release services include working with detainees with Opioid Use Disorder, drug and alcohol addiction, and have educational or housing needs. The Case Manager works with these individuals within the community with the resources to succeed. This includes ensuring that identification paperwork is in order, medical needs are being addressed, and Medicaid services are in place if appropriate.
- Required qualifications: must past a drug screening and background check. Must have a high school diploma or equivalent.
- Desired qualifications: Experience as a case manager working in the social services industry and OR Education in the social services industry such as an associate or bachelor's degree in social equivalent.
- Coordinate services with community resources, healthcare providers, social service agencies, and workforce development programs.
- Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies.
- Advocate for clients’ needs within various systems.
- Monitor client progress regularly and adjust care plans accordingly to ensure continuous improvement.
Skills
- Strong crisis intervention and crisis management skills to handle high-pressure situations effectively.
- Experience or knowledge in addiction counseling, behavioral health, social work, child welfare, and workforce development is highly desirable.
- Proficiency in motivational interviewing techniques to foster client motivation for change.
- Familiarity with programs such as, Medicaid, and other social services systems.
- Excellent communication skills with the ability to build rapport with diverse populations.
- Organizational skills for managing multiple cases simultaneously while maintaining accurate documentation.
- Compassionate approach combined with strong problem-solving abilities to support clients through complex challenges. This role is integral to empowering individuals toward healthier lives and stronger communities through dedicated case management services.
Job Type: Full-time
Pay: $22.00 - $23.00 per hour
