Hospice Liaison

Full-Time | Northern New Hampshire Territory

We are seeking a relationship-driven and mission-focused Hospice Liaison to serve as an ambassador for our hospice program and help patients and families navigate their healthcare journey with compassion and confidence.

This position plays a vital role in building strong partnerships with healthcare providers and referral sources while ensuring smooth transitions to hospice care. The ideal candidate is passionate about serving others, excels at developing relationships, and understands the unique value hospice care brings to patients and families.

What You'll Do

• Develop and maintain strong relationships with hospitals, physician offices, skilled nursing facilities, rehabilitation centers, and other community partners.
• Promote awareness and understanding of hospice services through education and outreach.
• Coordinate patient referrals and collaborate closely with internal teams and referral sources to ensure seamless transitions of care.
• Meet with patients and families to provide information and support regarding hospice services.
• Identify opportunities to expand community partnerships and grow referral relationships.
• Conduct regular visits throughout the northern New Hampshire territory, including communities such as Wolfeboro, Ossipee, and surrounding areas.
• Work collaboratively with the Hospice Director and Advancement team to support program growth and community engagement.
• Serve as a trusted resource for healthcare professionals, patients, and families.

What We're Looking For

• Sales or marketing experience required.
• Hospice or palliative care experience preferred.
• Bachelor's degree preferred.
• Nursing or social work background is a plus, but non-clinical candidates with hospice sales experience are encouraged to apply.
• Excellent communication and relationship-building skills.
• Strong organizational abilities with the capacity to manage multiple priorities.
• A compassionate, professional, and service-oriented approach.
