What are the responsibilities and job description for the Registered Nurse -- Community Transitions position at Cornerstone Services Inc.?
General Functions: Registered Nurse provides services, consultation, and guidance to staff and participants with regard to behavioral health programs. Assist PMHNP with nursing assessments and coordination of medical needs of clients transitioning into the community.
Specific Functions:
RN will offer consultation and guidance to staff with regard to physical or mental health concerns for individuals served.
Assist team in developing Comprehensive Service Plans based on the assessment and coordinating care and services for individuals transitioning to the community.
Provide transition preparation services to individuals still residing in SMHRF/NF as identified in their Comprehensive Service Plan.
Ensure continuity of healthcare/services and medical appointments.
Arrange for durable medical equipment and Home and Community-Based waiver services with Medicaid Managed Care health plan support, as appropriate.
Facilitate transfer of medications and linkages with primary healthcare, medical specialists, pharmacies, managed care organizations, and ancillary services/supports, as appropriate.
RN will complete nursing assessments, including review of medical, clinical charts and/or other pertinent documents as well as interviews with the individuals, family members, guardians, and/or significant others, as appropriate, about past and present functional levels, capabilities and performances, including prior community-based experiences and circumstances of admissions to SMHRF/NF, and complete narrative reports of these assessment outcomes with recommendations
RN will follow up within 7 days of an unscheduled ER visit or hospitalization and document.
Nurse will assist in identifying and setting up any appointments with specialists as needed.
Offer consultation in the development of program policies and procedures regarding health maintenance issues.
Documentation consultations and services with regard to both individual participants and the overall program.
RN will monitor the distribution of medications and follow-up on medical problems/hospitalizations.
RN will participate as needed in Psychiatric Review meetings.
RN will administer injections as prescribed, as needed.
Meet or exceed MRO billing quota as set by the department director.
Assist coverage of other nursing absences in behavioral health and community transitions absences to ensure appropriate client care.
Provide mental health services including:
Individual and Group Counseling
Crisis Intervention
Case Management
Client-Centered Consultation
Psychotropic medication administration
Psychotropic medication training
Community Support Individual
Travel to various program locations as assigned.
Maintain ongoing compliance with all agency-related training and certification requirements.
Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
Operate agency or personal vehicle in accordance with established policies and procedures.
Other duties as assigned.

Qualifications: Licensure as a Registered Nurse, one year's experience, knowledge of pharmacology. Psychiatric experience required. Driving record and auto insurance in compliance with agency policy. A post offer physical and criminal background check required. Reliable transportation required. EOE.
