What are the responsibilities and job description for the Care Manager-South Holland position at Cornerstone Services Inc.?
General Functions: Serve as an integral member of the agency's Williams Transitions Team. Primary functions: Conducting the screening and admission process, completing assessments and intakes, and developing comprehensive service plans to support individuals transitioning from Specialized mental health rehabilitation facilities (SMHRF) to the community. Your primary responsibility will be to actively work with Class members to achieve their goals both during their stay in the facility and while monitoring their post-transition progress.
Specific Functions:
Maintain release of information (ROI) for all collateral contacts during assessments, obtaining necessary signatures from nursing home facilities, class members, and/or guardians, if applicable.
Utilize the Webapp to complete required documentation for all individuals transitioning from SMHRF into the community.
Conduct background checks through Cornerstone Services and other sites recommended by UIC and the state.
Coordinate with SMHRF staff, class members, Managed Care Organizations (MCO), and other care team members to establish the appropriate level of care, needs, and goals.
Schedule neuropsychological evaluations and DON (Determination of Need) score assessments as necessary.
Arrange and coordinate required assessments within specified timeframes, ensuring accurate tracking and completing quarterly compliance.
Complete all necessary assessments, data tracking, report generation, required training, and visits to SMHRF, and provide transportation for individuals during the transition coordination period.
Utilize the assessment process and comprehensive service plan to identify all support and service needs for successful transition. Connect class members to all support in the community whether they are living independently and being served by cornerstone post transition or another agency out of our network. Search and complete the referral process for higher levels of care such as SLP?s, senior livings, or group homes.
Develop comprehensive service plans for class members transitioning into the community at the time of referral, during transition, and 180 days after transition.
Follow up with individuals, providing ongoing support, support services related to their service plans, and linking them to pre-transition services and resources as identified.
Develop, coordinate, and document individual transitions using established documents, timeframes, and criteria.
Provide comprehensive case management services and transition supports to facilitate successful movement of persons with mental illness out of SMHRF into the community.
Assist in the search for group homes with appropriate support and level of care in the geographic areas desired by individual class members.
Use a team approach to collaborate with supervisors and appropriate managers to communicate the results of the screening and intake process.
Participate in weekly team meetings and monthly clinical supervision meetings, seeking ongoing clinical supervision from supervisors, and engage in continuing education to maintain relevant licenses/certifications.
Maintain your weekly quota for MRO billing.
Maintain IM CANS certification and complete assessments and re-assessments every six months.
Facilitate agency training sessions and actively participate in agency committees as assigned.
Maintain a driving record and automobile insurance in compliance with agency policy.
Meet or exceed assigned weekly billing quotas as determined by the Director of Professional Services.
Participate in all agency and department training courses, meetings, online trainings, and any additional recommended trainings or meetings as required.
Ensure compliance with all certification requirements, including First Aid/CPR, Safety Care, etc.
Maintain ongoing compliance with all agency-related training and certification requirements.
Follow established procedures to respond to emergency situations, including medical, behavioral, fire, severe weather, and other hazards.
Operate agency or personal vehicles in accordance with established policies and procedures.
Own, maintain, and use personal smartphones for company business in accordance with established policies and procedures.
Perform any other duties as assigned.
Maintain ongoing compliance with all agency-related training and certification requirements.
Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
Operate agency or personal vehicle in accordance with established policies and procedures.

Qualifications: Qualified Mental Health Professional preferred (master's degree in counseling, social work, or related field with practicum and 2 years of experience under the supervision of a Qualified Mental Health Professional). Qualifications consistent with a Mental Health Professional will be considered in conjunction with prior clinical experience. CADC and DASA experience strongly preferred. Driving record and auto insurance coverage in compliance with agency policy. A post-offer physical and criminal background check required. Reliable transportation required. EOE
