Job Summary:

The Case Manager (SUD) is responsible for providing comprehensive case management services to individuals struggling with substance use disorders. The case manager works closely with a multidisciplinary team to develop, implement, and monitor individualized treatment plans, facilitate group educational sessions, and provide ongoing support to clients and their families.

Responsibilities:

· Conduct comprehensive assessments to determine the needs and priorities of individuals with substance use disorders.

· Develop, implement, and monitor individualized treatment plans based on clients' needs and goals.

· Maintain accurate and up-to-date case records, documenting all services provided, changes in clients' conditions, changes to treatment plans, and interactions with clients in company’s EMR system (KIPU).

· Educate clients and their families about substance use disorders, treatment options, and community resources.

· Regularly review and update treatment plans to reflect clients' progress and changing needs.

Minimum Qualifications:

• CDAC-l or CDAC- ll (CCAPP Certification), or equivalent.
• 2 years of case management experience in mental health, substance abuse, or related field.
• Bachelor’s degree in psychology, sociology, or related field (preferred).

· Excellent interpersonal and communication skills, with the ability to build strong relationships with clients, families, and other service providers.

· Ability to handle high-stress situations and make critical decisions under pressure.

· Strong organizational skills, with the ability to manage multiple cases and prioritize tasks effectively.

· Proficiency in electronic health records systems and Microsoft Office Suite.

• EMDR training and/or certification is a plus.
• Valid California Driver’s License required.
• Individuals in personal recovery must have a minimum of 2 years clean and sober.
• Must pass a criminal background check as required by licensing authorities.
• CPR, AED & First Aid Certification required.
• Annual TB Testing required.

Regulatory & Compliance Expectations:

• Maintain compliance with all state and federal regulations, including DHCS and Joint Commission standards.
• Follow company policies on safety, ethical conduct, and confidentiality.
• Participate in audits, training, and continuous quality improvement efforts.

Compensation & Benefits:

• Compensation Range: $23 - $30 per hour (commensurate with experience and qualifications).
