What are the responsibilities and job description for the Case Manager position at Cornerstone of Grace?
Purpose
The purpose of this position is to provide oversight to each family at Cornerstone of Grace. The Case Manager will be responsible for seeing that each participant’s requirements are met and coordinate services for them.

Work to be accomplished
- Assist with phone screenings and initial interviews of potential program participants.
- Provide one or more goal setting meetings weekly with each woman.
- Work with a team to identify personal barriers and develop an individualized plan.
- Ensure that each family is enrolled in required supports such as counseling, budgeting, and parenting classes and school/daycare.
- Conduct regular drug/alcohol screenings.
- Handle Intake paperwork and the assimilation process for each new program participant.
- Handle Exit paperwork for each program graduate.
- Handle Tier 2 paperwork for each program graduate.
- Support volunteers by communicating each woman’s general goals and communicating when there are changes to the schedule.
- Keep office space neat and tidy.
- Actively participate in all weekly staff meetings.
- Ensure that homes are locked and secure and that security equipment is working properly.
Key working relationships
The Case Manager(s) will collaborate internally with the part-time employees (Evening House Manager/and Program Staff). They will report to and be supported by the Executive Director and the Board of Directors. The Case Manager will attempt to be transparent about their own needs, the needs of the women served, and the needs of the organization maintaining the utmost confidentiality.
Qualifications
- A strong personal Christian faith, modeling the principles of Scripture in actions and words.
- Able to work with a team and independently
- Must be available to work flexible hours, including some weekends, evenings, and on call hours
- Good communication skills to empower and support staff and participants
- Ability to work with diverse populations
- Highly organized and have the ability to multitask
- Must have the ability to be exposed to and work around various allergens in the homes, including nuts.

Hours/Pay
- The Case Manager will work 25-35 hours each week with some flexibility
- Pay will be $18.00-$22.00 an hour
