Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
- Perform specialized procedures such as catheterization, venipuncture, and sterile dressing changes with precision and adherence to aseptic technique.
- Manage airway and ventilator support for patients requiring respiratory assistance while monitoring for signs of distress or complications.
- Document all patient interactions thoroughly using electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to ensure accurate medical records.
- Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services.
- Provide triage assessments in urgent care or emergency room settings to prioritize patient needs effectively.
- Support patients with developmental disabilities, dementia, Alzheimer’s disease, and memory care needs through specialized approaches that promote safety and dignity.
- Assist with diagnostic evaluations involving medical imaging or laboratory specimen collection/processing to facilitate timely diagnoses.
- Educate patients and families on health coaching topics such as nutrition, medication administration, pain management, and chronic disease management.
- Maintain compliance with HIPAA regulations and infection control standards to safeguard patient confidentiality and safety.
Skills
- Proven experience working with EMR/EHR systems like Epic, Cerner, eClinicalWorks or Athenahealth for efficient documentation.
- Strong critical care background including ICU experience or Level I/II trauma center exposure is highly desirable.
- Expertise in various clinical areas such as medical-surgical nursing, geriatrics, pediatrics—including neonatal ventilator management—and behavioral health settings.
- Skilled in performing invasive procedures like phlebotomy, IV insertion/infusion therapy, catheter care, spinal taps, gastric lavage, and dialysis support.
- Knowledge of advanced physiology concepts related to patient assessment and diagnostic evaluation across diverse populations.
- Familiarity with ICD coding (ICD-9/10), CPT coding for procedures, and documentation standards aligned with NCQA or hospital accreditation requirements.
- Ability to work effectively with patients experiencing acute pain or complex conditions such as infectious diseases or post-operative recovery.
- Experience providing compassionate care in assisted living facilities or home & community care environments is a plus.
