Cornerstone Behavioral Healthcare is seeking a compassionate, motivated Health Home Care Coordinator/Case Manager to join our Bangor team. As a Behavioral Health Home (BHH), we provide culturally responsive, integrated, and trauma-informed care in a supportive, collaborative environment.

Position Overview

As a Health Home Care Coordinator/Case Manager, you'll play a vital role in supporting adults living with Serious and Persistent Mental Illness (SPMI). You'll work directly with clients to develop, implement, and oversee individualized Plans of Care while coordinating services, advocating for client needs, and helping individuals achieve their personal recovery goals.

Key Responsibilities

  • Provide care coordination services for individuals with co-occurring mental health and substance use disorders.
  • Complete and coordinate comprehensive biopsychosocial assessments.
  • Develop, implement, and oversee individualized Plans of Care in collaboration with clients.
  • Coordinate care with providers to support integrated behavioral and physical healthcare.
  • Assist with crisis planning and intervention.
  • Refer clients to community resources and support services.
  • Advocate for client rights and assist with transitions between levels of care.
  • Participate in multidisciplinary team meetings with clients, families, and community providers.
  • Accompany clients to medical, behavioral health, and community appointments as needed.
  • Maintain timely and accurate documentation in the electronic health record.
  • Submit required authorizations and reports in compliance with MaineCare and DHHS guidelines.

Qualifications

Required

  • MHRT/C certification preferred. Candidates eligible for Provisional MHRT/C certification are encouraged to apply and must complete certification requirements within one year of hire.
  • Valid Maine driver's license and current automobile insurance (minimum $100,000/$300,000 coverage).
  • Ability to travel to client homes and community settings, including locations requiring the use of stairs.
  • Ability to sit or remain in one position for up to 50% of the workday.
  • Proficiency with computers, electronic health records, and standard office equipment.
  • Excellent communication, organization, time management, and problem-solving skills.
  • Ability to maintain confidentiality and professional boundaries.

Preferred

  • One to two years of experience working with individuals living with mental illness (we are willing to train the right candidate).
  • Familiarity with LOCUS assessments and the needs of individuals with SPMI.
  • Ability to work effectively within a multidisciplinary team.

Pay: $22.00 - $24.00 per hour
