Position Overview:
As a Case Manager/Health Home Coordinator, you’ll play a vital role in supporting adults with Serious and Persistent Mental Illness (SPMI). You’ll work directly with clients to create, implement, and oversee individualized Plans of Care while coordinating services and advocating for client needs.

Key Responsibilities:
- Engage and serve individuals with co-occurring mental health and substance use disorders
- Complete and/or coordinate comprehensive bio-psychosocial assessments
- Develop and oversee individualized Plans of Care in collaboration with clients
- Collaborate with Primary Care Providers to integrate behavioral and physical health care
- Support crisis planning and resolution
- Refer and link clients to needed resources and supports
- Advocate for client rights and assist with transitions between care levels
- Participate in regular team meetings with clients, families, and service providers
- Provide transportation and support for community-based activities
- Maintain accurate documentation in the Electronic Health Record system
- Submit required authorizations and reports in compliance with MaineCare and DHHS guidelines
Qualifications:
Required:
- MHRT/C or Provisional MHRT/C Certification
- Valid driver's license and current vehicle insurance (100,000/300,000 coverage)
- Ability to visit clients in home and community settings, including the use of stairs
- Ability to sit or remain in one position up to 50% of the day
- Comfort with basic office equipment and electronic systems
- Strong communication, time management, and problem-solving skills
- High level of confidentiality and professional boundaries
Preferred:
- 1–2 years of experience working with individuals with mental illness (willing to train the right candidate)
- Familiarity with LOCUS and the needs of SPMI clients
- Willingness to work within a multidisciplinary care team
Job Type: Full-time
Pay: $22.00 - $23.00 per hour
