Job Summary
The Health & Justice Program Coordinator supports justice-involved youth and young adults through individualized case management, group facilitation, and direct community engagement. This hands-on role involves leading multiple life skills groups each week, conducting initial contacts and regular client check-ins, developing and updating Individualized Service Plans, and working closely with schools, courts, and providers to ensure coordinated, trauma-informed support. The ideal candidate will be deeply rooted in community-based work, highly organized, creative, and committed to walking alongside young people with consistency and care. This is a full-time, 40-hour/week position with some evening or weekend hours required.
Essential Duties and Responsibilities
- Lead multiple weekly life skills and emotional wellness groups for youth ages 12–25.
- Conduct regular client check-ins, provide hands-on support, and assist in developing individualized service plans.
- Communicate using a client-centered, trauma-informed, and judgment-free approach.
- Coordinate and track referrals, appointments, and case updates with internal and external providers.
- Maintain accurate and timely documentation using electronic case management systems.
- Support clients in navigating barriers like transportation, court obligations, or accessing services.
- Attend court sessions and contribute to community-based outreach, home visits, or follow-ups as needed.
- Collaborate with internal teams and community partners to ensure holistic and culturally responsive care.
- Participate in team meetings, case reviews, and program evaluation activities.
- Assist the Program Manager with administrative tasks and special projects as needed.
- Provide feedback and suggestions to improve referral and assessment processes.
- Provide services in accordance with the MDHHS CAHC Minimum Program Requirements and grant guidelines.
- Commitment to the agency’s mission and initiatives.
- Maintain client confidentiality.

Qualifications:
- Bachelor’s degree in social work, psychology, criminal justice, youth development, or a related field required.
- Candidates with an associate degree and 3 years of relevant experience will be considered.
- 2–3 years of experience facilitating youth-centered groups, especially around life skills, behavior change, or mental wellness.
- 2 years of case management or direct support work, ideally with justice-involved or at-risk youth.
- Strong relationship-building and community engagement skills, with an ability to support youth in diverse settings.
- Knowledge of and commitment to trauma-informed care and youth development best practices.
- Creative thinker who brings fresh ideas to group facilitation, youth engagement, and community connection strategies.
- Organized, detail-oriented, and able to balance multiple priorities.
- Excellent written and verbal communication skills.
- Ability to work both independently and as part of a collaborative team.
- Experience using Casebook or Athena is preferred but not required.
- Proficient in Microsoft Office; must have reliable transportation and a valid driver’s license.
Work Environment:
- Based in a community health setting and involves direct work with youth and young adults (ages 12–25).
- Evening and occasional weekend hours may be required.
- Local travel may be required; mileage is reimbursed.
- Must have access to reliable transportation.
- Role involves sitting, standing, walking, and light lifting (up to 25 lbs).
Salary range for this position is $45,000-$50,000- depending on experience
