Title: Clinical Nurse Coordinator (LPN)Department: Homeless Department Reports To: Program Manager - Homeless HealthcareSummary The Clinical Nurse Coordinator supports the SHOUT (Street HIV Outreach to Engage Unsheltered Populations in Treatment) program and the SPNS Grant (Special Project of National Significance) by providing direct clinical care, street-based outreach, and data management for grant reporting. This role focuses on identifying, testing, and linking unsheltered individuals to care for HIV, HCV, STIs, and other health conditions, while also documenting and evaluating best practices in street medicine.The Clinical Nurse Coordinator must be a compassionate, detail-oriented clinician with experience in outreach or community health. Must be comfortable working outdoors in all weather conditions and able to work flexible hours, including early mornings and evenings. Travel is required to participate in regional and national meetings and to present program data. Active Connecticut LPN license and valid driver’s license required.CS-HHC’s Expectations of all Employees Adheres to all CS-HHC Policies and Procedures Conducts self in a manner that represents CS-HHC’s Values at all times Maintains a positive and respectful attitude with all work-related contacts Communicates regularly with supervisors about Departmental and CS-HHC concerns Consistently reports to work on time, prepared to perform the duties of the position Meets productivity standards and performs duties as workload necessitates Establish relationships with collaborating organizations and volunteer groups for optimal Communication in the care of our patients.Essential Duties and ResponsibilitiesOutreach and Clinical CareConduct field-based outreach in the Greater New Haven region, engaging unsheltered individuals for screening, prevention, and treatment of HIV, HCV, and other STIs.Provide infectious disease education and overdose prevention education tailored to patient and community needs.Perform point-of-care testing and screenings following established clinical protocols.Administer medications and assist with wound care procedures under provider direction.Support implementation of treatment plans, ensuring adherence, and follow-up.Maintain accurate clinical and program documentation within the electronic health record (EHR).Participate in early morning, evening, and weekend outreach as needed (minimum two early and two evening shifts per week).Operate and maintain the medical van, ensuring safety, cleanliness, and readiness for outreach.Care Coordination and DocumentationCollaborate with providers to maintain accurate registries of unsheltered patients receiving care.Document all clinical encounters, referrals, and follow-up in the Electronic Health Record (Epic) per program standards.Collect and analyze program data and support the implementation of evidence-based interventions that improve care outcomes for people experiencing homelessness.Ensure data quality for all client-level and service-level reporting.Maintain logs for point-of-care testing, including control results and expiration tracking.Assist providers with administrative tasks and visit preparation to ensure workflow efficiency.Grant and Data ManagementCollect and maintain accurate program data for grant reporting, including client demographics, testing outcomes, and service delivery metrics.Identify and document emerging best practices in outreach and engagement for unsheltered individuals with HIV.Evaluate street medicine interventions and contribute to the development of data-informed program improvements.Collaborate with the Program Coordinator to prepare required data summaries and reports for funders.Travel may be required to participate in regional and national meetings and to present program data.Collaboration and AdvocacyBuild and sustain strong partnerships with community agencies and healthcare providers.Attend and participate in inter-agency meetings to foster coordinated care approaches.Advocate for clients to access medical, behavioral health, and social services.Ensure strict confidentiality and compliance with HIPAA, state, and agency requirements at all times.Qualifications Education: Must have a valid CT LPN license in good standing. Current BLS required. Experience: Minimum of two years of clinical experience, preferably in community health, infectious disease, or homeless healthcare. Experience with outreach, harm reduction, or public health preferred.Knowledge/Abilities: Proficient in documentation using Electronic Health Records (Epic preferred). Comfortable working outdoors in all weather conditions. Excellent organizational skills with strong attention to data accuracy and reporting. Strong interpersonal skills and ability to engage vulnerable populations with compassion and professionalism. Valid driver’s license and reliable transportation required. Able to work unconventional work hours, including evenings and weekends.CS-HHC is an equal-opportunity employer.
