Title: Infectious Disease APRN/PA
Reports To: Director of ID
Summary
Provides reasonable and prudent medical care, practices appropriate management and uses sound clinical judgment based in best practice standards and evidenced based medicine. The Physician provides medical services within the scope of their licensing, training and credentialing.
Essential Duties And Responsibilities
Education: Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school. CPR Certification required.
Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one’s hire.) Connecticut Licensure. Must have at least 5 years of clinical experience in outpatient internal medicine or family practice. Prior experience working in addiction medicine preferred.

Essential Duties And Responsibilities
- Conducts comprehensive medical evaluations, education, and treatment.
- Maintains patient confidentiality in compliance with agency, state, and HIPAA laws.
- Assesses medical and psycho-social histories, performs physical exams, identifies problems, formulates treatment plans, provides education and referrals, and evaluates intervention effectiveness.
- Orders and interprets laboratory tests, reports communicable diseases to the Health Department as per protocol.
- Prescribes appropriate medication.
- Implements standardized evaluations or assessments with a multidisciplinary team.
- Ensures complete, timely, and accurate documentation.
- Understands medical emergency procedures and the location of emergency supplies.
- Participates in continuing medical education to enhance professionalism and quality of care.
- Provides on-call coverage as scheduled.
- Maintains good working relationships with community agencies.
- Attends and participates in team conferences, huddles, and required trainings.
