LMSW
- Support patients and families in navigating behavioral health services and resources across acute and community settings, using in-person, electronic, and telephonic communication to bridge care gaps.
- Conducts comprehensive assessments to determine health requirements, care preferences and social determinants, providing navigational support to facilitate appropriate care solutions.
- Utilize clinical interviewing skills to elicit patient information and determine appropriate interventions.
- Accurately completes and maintains clinical documentation in the electronic medical record, reflecting assessments, care plans, and transition needs.
- Provides consultation and resources to members of the healthcare team on behavioral health placement options and care coordination strategies.
- Serve as a liaison between patients, families, healthcare providers, and external agencies to ensure continuity and clarity of care delivery.
- Deliver short-term therapeutic assistance suited to the specific setting and location.
- Stay informed on current community resources, behavioral health regulations, and participate in relevant committees or initiatives that support role development internally and externally.
- Required Master's Degree Social Work
- 3 years of relevant experience Experience in health care following completion of a Master’s Degree in Social Work, with a focus on care management preferred. Required
- Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning. Preferred
- Master Social Worker (MSW-Master) LMSW - State of Michigan Upon Hire required
