About Corewell Health South Hospice

We are a community based non-profit organization serving 3 counties in Southwest Michigan: Cass, Berrien, and Van Buren for over 30 years. We specialize in providing end of life care and symptom management to individuals with a life expectancy of six month or less. We are one of the few Hospices in Southwest Michigan with a 16-bed inpatient hospice residence to serve patients and families needing this environment.  We serve individuals in their home, nursing facilities, and in the hospital.

Scope of Work

The RN is responsible for assessing, planning, implementing, coordinating, monitoring and evaluating the strategies and services required to meet the patients/family's end-of-life care needs. An outcome based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death. Exhibit “Patients and Families First” attitude. Responds with compassion, flexibility and urgency to each patient need. Provides the care that patients want, when they want it.

Qualifications

Required

• Associate’s degree in nursing

• 2 years of relevant experience

• Michigan Registered Nurse (RN) license upon hire

• State of MI Driver’s License upon hire  

• AHA or ARC Basic Life Support (BLS) within 90 days

Preferred

• Bachelor’s degree in nursing

• 1 year of relevant experience of acute care, emergency, or rehab facility experience for all of the Home Care business lines
