About Us : Our Visiting Nurses Association represents our Home Care Skilled Nursing Team, dedicated to serving communities across West-Central Michigan. We are looking for passionate LPNs, whether you are a seasoned professional or a new graduate. This role provides you the freedom to apply your nursing expertise and critical thinking skills in the comfort of patients' homes, helping them to take charge of their health. You will manage a manageable caseload of 5-7 patients per shift, enabling personalized care close to home. Our service area includes Ludington, Baldwin, Nunica, and down to Gun Lake in Kent County and Ottawa County.
Position Overview : As an LPN, you will work under the guidance of an RN, Mid-level Provider, or Physician, collaborating with a healthcare team to assess, plan, implement, and evaluate patient care. You’ll provide vital information to patients and families about treatments, medications, and equipment used during care in conjunction with the RN or physician. Documentation of care will be required through both written and computer records.
