Job Summary - Director, Health Equity and Comm Health

The Director of Health Equity and Community Health Programs reports to the Vice President of Patient and Family Experience and Health Equity. In this role, you will be responsible for advancing Beaumont Health's community health programs, community outreach, and health equity strategies. You will lead the development and implementation of the federally mandated Community Health Needs Assessment (CHNA) which is completed every three years and implemented ongoing.
Additional responsibilities include:
-Leading programs and functions to address community need as well as partnering with internal operations and external entities.
-Critical identification and tracking of community benefits provided by Beaumont to improve the health of the community.
-Strategic direction and operational management of community benefit compliance, fiscal management, community engagement, and annual work plan development associated with health equity and community health programs.
-Ensures alignment of community programs with the priority health needs identified in the Beaumont Health CHNA, by developing evaluation metrics, tools, and outreach strategies, identifying, and securing new funding opportunities and engaging and educating system leaders to ensure compliance and reporting of system community benefit.
-Lead relationships with numerous external stakeholders and internal partners to meet the community needs and ensure Beaumont Health is engaging in programs that promote health equity in our communities.
-Provides expertise and guidance in matters relating to community health improvement and ensuring desired outcomes are achieved.

Essential Functions - Director, Health Equity and Comm Health
  • Provides system leadership and strategic direction for Beaumont Health CHNA and leads system Implementation Plan strategies to meet mandated IRS 501(r) regulations for nonprofit hospitals. Engage regularly with leadership in finance, compliance, and government relations.
  • Leads community health programs.
  • Leads Health Equity initiatives across Beaumont Health.
  • Responsible to partner with the Director of Health Equity and Community programs to ensure alignment of strategies and achievement metrics.
  • In collaboration with system leaders (i.e., strategy, population health, quality) develops a portfolio of solutions to help meet system and regional, and state level population health goals, specifically those focused on reducing health disparities, structural barriers to health and improving outcomes for BIPOC, LGBTQIA and other marginalized populations.
  • In collaboration with internal and external partners, design and build a system-level Health Equity function to support large, complex, strategically essential, cross-functional, and cross-regional initiatives that directly address the commitment to achieving health equity.
  • Serve as a conduit between entities, system community benefit and health equity leaders and data analytics, EPIC, and business intelligence teams to identify appropriate means of measuring impact of key initiatives and publish a system Health Equity Dashboard. Work collaboratively to ensure a regular cadence of reporting to measure progress and support pivots as needed.
  • Work with system, regional, local community, and health equity leaders to proactively plan for sustainability of key initiatives, including identification of federal funding mechanisms, collaboration with payer partners and advocacy opportunities.
  • Leads development of required IRS Community Health Needs Assessment and Implementation Strategy process for the eight Beaumont Health hospitals, including development and engagement with leaders in marketing, hospital leaders, government relations, finance, planning, tax, compliance, and others. Ensures system-wide strategic alignment and operational excellence in community benefit administration, reporting, and compliance. This includes ensuring completion of community health needs assessment and plan every three years by engaging boards, leaders and external stakeholders in understanding findings and priorities.
  • Implements new and emerging models aligned with system priorities, through the identification of external funding opportunities through the development of external grants in partnerships with BH Foundation Vice Presidents and community partners for community health and CAHC grants.
  • Leads/manages community health program managers and establishes performance goals, allocation of resources and policies. Hires new staff as needed. Provides orientation, training, and continuing education of employees in area(s) of responsibility.
  • Leads hospital or facility community benefit managers in assessing their community’s health needs and assets; developing and managing community outreach initiatives; and measuring and reporting program accomplishments and results.
  • Develops, operationalizes, and evaluates the financial impact of initiatives and programs that leverage collaborative partnerships with community organizations, government agencies and health plans to mitigate barriers for our patients in partnership/collaboration.
  • Creates new relationships and collaboratively nurtures existing alliances and partnerships with community organizations and local governments (i.e. MSUE, NKFM, city leadership, Oakland County Health Department, Wayne County Health Department) across Southeast Michigan to foster optimal community health outcomes.
  • Prepares written and verbal presentations of findings, conclusions, and recommendations to share with executives, Hospital Community Advisory Boards, Board of Director Reports, and in required IRS Form 990 Schedule H tax returns.
  • Performs all operational, financial, and administrative management activities pertinent to the role and scope of responsibilities.
  • Evaluates facility community benefit plans and reports ensuring compliance with BH policy and regulatory standards.
  • Provides administrative oversight and leadership of system community benefit, including ensuring accurate and timely submission of data for nonprofit tax compliance. Provides leadership and management of staff collecting community benefit data, ensuring adherence to system standards for calculating community benefit expenses.
  • Serves on internal leadership groups and committees.
  • Develops and maintains current knowledge of community benefit principles and practices Informs/educates appropriate Beaumont Health representatives of current trends.
  • Develops pertinent background research which contributes to a greater understanding of best practices, issues and trends impacting assigned programs, subjects, and topics.
  • Provides leadership to administrative leaders, physicians, other professional staff, and community leaders on program/project evaluation, including the development of data collection instruments, databases, community health metrics and the creation of evaluation reports for community benefit/health programs.
  • Monitors progress and provides regular reports of hospital community benefit and system goals to leaders.
  • Ensures administrative management, collection, review, and submission of IRS Form 990 Schedule H narrative responses to tax to meet regulatory and filing requirements.
  • Develops and manages grants and contracts in Meditract, including flow through of agreements in partnerships with legal and compliance for community partners (i.e. The Senior Alliance, MDHHS, Michigan Fitness Foundation).
  • Provides ongoing support in the development of grants in partnership with Director, Child, and Adolescence Health Centers, including evaluation of programs and creation of reports to funders.
  • Develops annual department budget on behalf of Vice President, in partnership with Director, Community Health Clinics and Operations.
  • Develops regular stewardship reports for donor-provided funds (i.e., Kosch Grant). Manages relationship and ensures completion of program related activities.
  • Regularly meets and interfaces with Government Affairs team, including VP Government Affairs and Government Affairs Managers to ensure communication and alignment of efforts across all BH communities.
  • Primary presenter for BH Community Advisory Boards for each hospital community representing Community Health.
  • Develops annual Community Benefit Report as required by law.
  • Directs multiple teams developing and delivering health promotion and community outreach activities including budget management, grant administration, evidence-based program development, inclusion of appropriate liability protections in program implementation, and reporting on outcomes and customer satisfaction.

Qualifications - Director, Health Equity and Comm Health
  • Required Master's Degree Public Health, Social Work, Business, or another related field
  • 10 years of relevant experience in a management role with proven track record of effectively leading a performance-based and outcome-based program and staff Required
  • Demonstrated experience developing and operationalizing strategies that have taken a program or organization to the next stage of growth.
  • Experience in recruiting, managing, and developing a strong team of staff and program/project managers.
  • Innovative thinker, with a track record for translating strategic thinking into action plans and output.
  • Demonstrated experience in grant writing/fundraising experience with the ability to engage a wide range of stakeholders including advisory groups, coalitions, foundations, corporate sponsors, and physicians.
  • Exceptional written, oral, interpersonal, and presentation skills and the ability to effectively interface with senior management, the Board of Directors, physicians, and staff.
  • Ability to work effectively in collaboration with diverse groups of people.
  • Demonstrated knowledge in working with community health issues, ability to interact across multiple acculturation levels and socio-economic groups.
  • Strong decision-making ability.
  • Well organized; able to manage and prioritize changing workloads and meet deadlines.
