Position Overview:
As a Client Care Coordinator, you will be responsible for creating and managing patient care plans from intake to completion. Your role ensures patients feel cared for, treatment plans are followed, and all services are provided efficiently and ethically. This includes working with diverse payer types and collaborating with internal teams and external referral sources to deliver excellent care while securing timely payment for services.
Key Responsibilities:
Care Plan Development and Oversight:
- Draft detailed care plans for all patients, regardless of insurance type, outlining diagnoses, goals, services, and benchmarks for progress.
- Collaborate with multidisciplinary teams to ensure holistic care tailored to individual needs.
- Educate patients on their care plans, ensuring they understand treatment goals, responsibilities, and timelines.
Cost Calculation and Presentation:
- Calculate care plan costs, breaking down services by CPT codes, payer policies, and patient responsibilities.
- Present itemized cost breakdowns to patients and stakeholders in a clear, professional manner.
- Manage pre-authorizations for commercial, Medicare, workers' compensation, and MVA cases to ensure proper coverage.
Patient Intake and Coordination:
- Handle patient intake across all payer types, collecting demographic and insurance information.
- Manage initial assessments and set up care plans in the electronic health record (EHR) system.
- Serve as the central point of contact for patients and referral sources, ensuring a smooth onboarding process.
Progress Tracking and Compliance Management:
- Use tools like EHR alerts and outcome measures to monitor patient progress and identify gaps in care plan adherence.
- Ensure all appointments, make-ups, and follow-ups are scheduled and completed.
- Document milestones and barriers, adjusting care plans as necessary to meet patient needs.
Timely Submission and Payment Management:
- Submit claims and supporting documents for all patient encounters within 24 hours of service.
- Track the status of pre-authorizations, approvals, and claims to avoid delays in service or payment.
- Communicate with funding companies, payers, and referral sources to resolve issues quickly and effectively.
Relationship Building:
- Act as the primary liaison for employers, adjusters, attorneys, and funding companies.
- Foster strong relationships with referral sources through consistent communication and problem-solving.
- Participate in meetings with referral partners to strengthen partnerships and improve processes.
Performance Objectives:
- Ensure patients remain compliant with their care plans and feel supported throughout their journey.
- Maintain timely communication with referral sources and internal teams to keep care plans on track.
- Track key performance indicators (KPIs) for patient care, referral management, and case progress.
- Provide timely and accurate documentation to all stakeholders.
