Cordial Health is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant (PA) with psychiatric experience to provide on-site behavioral health care throughout Buffalo, New York.
This full-time opportunity combines meaningful clinical work, provider autonomy and extensive support from an experienced behavioral health team. You will care for adult and geriatric residents whose psychiatric needs may otherwise go untreated or undertreated.
Responsibilities include:
- Conduct comprehensive psychiatric evaluations, consultations and follow-up visits
- Diagnose and manage psychiatric and behavioral health conditions
- Develop individualized, evidence-based treatment plans
- Prescribe, monitor and adjust psychotropic medications
- Evaluate treatment response, medication effectiveness and side effects
- Collaborate with physicians, nursing staff, therapists, social workers, residents and families
- Participate in interdisciplinary treatment planning
- Educate residents, caregivers and staff regarding diagnoses, medications and behavioral interventions
- Provide compassionate, trauma-informed and resident-centered care
- Maintain accurate, timely and compliant clinical documentation
