Psychiatric Mental Health Nurse Practitioner (Columbia/Ulster) Counties

We are expanding our behavioral health services and are seeking a compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to provide on-site psychiatric care within skilled nursing facilities throughout Columbia County and Ulster County, New York.

Requirements

• Complete comprehensive psychiatric evaluations, behavioral health consultations, and diagnostic assessments for adult and geriatric residents

• Establish psychiatric diagnoses and formulate evidence-based treatment plans tailored to each resident's behavioral, emotional, medical, cognitive, and psychosocial needs

• Provide ongoing psychopharmacology management, including initiating, adjusting, monitoring, and optimizing psychiatric medications

• Evaluate behavioral disturbances associated with dementia, depression, anxiety disorders, bipolar disorder, schizophrenia, psychotic disorders, sleep disorders, personality disorders, adjustment disorders, and chronic mental illness

• Assess residents experiencing cognitive decline, mood instability, agitation, aggression, hallucinations, or other neuropsychiatric symptoms

• Collaborate with attending physicians, nursing staff, therapists, rehabilitation professionals, social workers, case managers, and facility leadership to coordinate comprehensive behavioral healthcare

• Participate in interdisciplinary treatment planning and provide recommendations that improve resident outcomes while promoting behavioral stability

• Educate residents, family members, guardians, and facility staff regarding psychiatric diagnoses, medication management, behavioral interventions, and realistic treatment expectations

• Identify opportunities for early psychiatric intervention that help reduce unnecessary emergency department visits, psychiatric admissions, and avoidable hospital transfers

• Maintain timely, organized, and compliant documentation consistent with state, federal, facility, and organizational standards

• Promote a compassionate, resident-centered approach that emphasizes dignity, respect, safety, and continuity of psychiatric care

Qualifications

• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Physician Assistant with significant psychiatric experience

• Active and unrestricted New York license

• DEA registration or ability to obtain

• Strong knowledge of psychiatric assessment, diagnosis, psychopharmacology, and treatment planning

• Ability to independently manage a psychiatric caseload while collaborating effectively with interdisciplinary healthcare teams

• Previous experience in psychiatry, behavioral health, skilled nursing, long-term care, rehabilitation, geriatrics, or post-acute care preferred

• Experience managing dementia-related behaviors, mood disorders, anxiety disorders, psychosis, chronic psychiatric illness, and medically complex patients preferred

• Excellent communication, organizational, clinical reasoning, and relationship-building skills

• Genuine passion for serving elderly and underserved patient populations
