Role Description The Psychiatric Mental Health Nurse Practitioner (PMHNP) will provide comprehensive psychiatric evaluations, ongoing medication management, and evidence-based therapeutic interventions for residents in nursing homes. Day-to-day responsibilities include collaborating with interdisciplinary care teams, developing and updating individualized treatment plans, monitoring treatment effectiveness, and documenting care in accordance with regulatory and clinical standards. The PMHNP will educate patients, families, and facility staff on mental health conditions, treatment options, and strategies to support behavioral health. This is a full-time, on-site role based in New York, United States, requiring regular travel to partner nursing homes within the region.
Qualifications
- Strong foundation in Mental Health Nursing, with experience providing psychiatric support in long-term care or similar settings.
- Licensure and clinical competence as a Nurse Practitioner, preferably board-certified as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Demonstrated skills in Mental Health Care, including assessment, diagnosis, treatment planning, and patient education.
- Proficiency in Psychiatric Care, including psychopharmacology, crisis intervention, and management of complex comorbid conditions.
- Core Nursing skills, such as holistic patient assessment, care coordination, and adherence to ethical and professional standards.
- Active NP license in New York State and DEA.
- Excellent communication, documentation, and collaborative skills with patients, families, and multidisciplinary teams.
- Comfort working in nursing home environments and openness to integrating telehealth tools and workflows.
