ORGANIZATION BACKGROUND
Corazón Healdsburg is a nonprofit organization whose mission is to build a compassionate and just community by empowering and dignifying individuals and families, advocating against injustices, and uniting people in celebrating diversity.

We operate a bilingual resource center in Healdsburg and offer a wide range of family support programs, education for all ages, financial guidance, and emergency response services. Spanish is the primary language for many of the individuals we serve. We coordinate and amplify the local Latine voice and host cultural events that publicly celebrate the Latine community while building cultural bridges among neighbors through celebration and volunteerism.

Everything we do supports our goals of breaking cycles of poverty for participating families and empowering them to create new connections with one another, with the broader community, and with local systems and services meant to serve everyone.

SUMMARY OF THE POSITION: Corazón Healdsburg is hiring a bilingual (Spanish and English) full-time hourly Family Advisor who will execute the Holistic and Family Centered Case Management under Corazón Healdsburg’s Casa del Corazón. Under the supervision of the Resource Center Manager, the Family Advisor will primarily be responsible for providing day-to-day case management that connects Northern Sonoma County families and individuals to comprehensive wrap-around supportive services, and resources.

Casa del Corazón Services Include: Information & referral, case management & follow-Up, advocacy, language access & interpretation support, crisis intervention, co-located partner services, trainings, and evening workshops.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide resources and networks of support through various types of service connections, including basic and comprehensive information and referrals, family advocacy, case management, and follow-up services.
  • Maintain accurate and comprehensive case notes and service records for all client interactions using the organization’s cloud-based (Salesforce)
  • Incorporate into all case management practices a deep respect for cultural diversity, strengths, and individuality across all families, including ethnic, racial, spiritual, social, economic, educational, linguistic, gender, and geographical diversity.
  • Remain knowledgeable about available community resources across all service areas to provide information and referral services, including resources for farmworkers, seniors, undocumented residents, and immigrant families living in Sonoma County.
  • Partner with families to develop individualized plans of care and coordinate with community partners to create a menu of family support services that address the strengths and needs of the families and communities served.
  • Collaborate with other staff members to maximize program effectiveness where duties and scopes of work overlap (eg. Early childhood education, Advocacy).
  • Support the planning and facilitation of workshops, classes, and group sessions that respond to patterns emerging from the Family Resource Center.
  • Participate in professional development, training, and learning opportunities relevant to the role and recommended by the team.
  • Participate in team meetings, case conferences, and cross-department collaboration to strengthen communication, alignment, and teamwork.
  • Conduct community outreach to build awareness of available programs and services.
  • Represent Corazón Healdsburg at community events, workshops, and partner meetings.


REQUIRED QUALIFICATIONS:

  • Education & Experience: Two years of experience in non-profit case management and demonstrated experience working in a community-based setting.
  • Bilingual & Cultural Competence: Strong written and verbal communication skills in English and Spanish. Ability to provide culturally responsive services and navigate cross-cultural communication with diverse communities.
  • Personality & Professionalism: Independent thinker with a strong commitment to a non-biased, client-first model that prioritizes meeting community members where they are and supporting them with a culturally responsive approach. Maintains a professional and positive attitude, high personal integrity, and sound judgment. Team-oriented, collaborative, and dedicated to fostering a respectful, supportive, and positive workplace culture for both clients and staff.
  • Performance & Organizational Skills: Ability to multitask, prioritize, and meet deadlines while adjusting to emerging demands and trends from the community, with the ability to reassess and reconfigure strategies as needed. Skilled in maintaining confidential information and managing multiple responsibilities simultaneously. Demonstrates strong attention to detail and analytical thinking.
  • Problem-Solving & Flexibility: Creative problem-solver able to adapt to changing community needs, organizational priorities, and emerging challenges.
  • Community Engagement & Family Support: Deeply committed to empowering families through individualized and group-based support, including education, advocacy, and referrals. Ability to identify patterns and challenges in community needs and respond effectively.
  • Stress Management: Ability to remain tactful under pressure and present a professional demeanor when interacting with coworkers and clients in multicultural settings.
  • Technology: Proficiency in G-Suite applications. Experience with CRM systems, preferably Salesforce, and other cloud-based tools for case management.
  • Confidentiality: Ability to maintain the highest level of confidentiality when handling sensitive client information.
  • Decision-Making & Initiative: Ability to set priorities, establish standards of performance, and develop actionable plans to accomplish work goals with minimal supervision.

  • Requirements: Must successfully complete DMV and criminal background checks, including fingerprint clearance through the Department of Justice.


PHYSICAL DEMANDS
: The essential functions of this position may require the ability to sit at a desk and perform phone or computer work for extended periods, communicate verbally, and lift or move paperwork, files, and computer equipment up to 25 pounds. Employees may also be required to walk, sit, use hands to handle objects and operate keyboards, reach with hands and arms, stoop, kneel, crouch, talk, and hear, as well as walk up and down stairs. WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee may encounter while performing the essential functions of this job. This position generally maintains regular office hours at the Healdsburg Community Center, with occasional field work at off-site locations where accessibility may vary. The noise level is typically moderate. This role routinely uses standard office equipment, including computers, phones, photocopiers, filing cabinets, and fax machines. The position may require light travel to various locations throughout Sonoma County.
