Requirements
● Doctor of Physical Therapy (DPT) with independent state licensure
● NPI number (or willingness to obtain one)
● Minimum 1 year of pediatric experience in the past 5 years
● No CFs or assistance as we’re unable to provide the supervision needed through our care model
● Comfortable working independently in home and community settings
