Role Description The Case Manager role at COR Health is a full-time, hybrid position based in Portland, ME, with the flexibility to complete some responsibilities from home. The Case Manager coordinates and monitors services for a diverse caseload, including individuals receiving home and community-based supports. Daily responsibilities include conducting assessments, developing and updating person-centered care plans, and ensuring services align with regulatory and funding requirements. The Case Manager communicates regularly with clients, families, providers, and community partners to advocate for client needs, address barriers, and maintain continuity of care. Additional responsibilities include maintaining accurate documentation, tracking outcomes, participating in team meetings, and staying current with relevant state regulations and COR Health policies.

Qualifications

  • Experience in case management, social work, human services, or a related field, preferably in home and community-based settings.
  • Knowledge of Maine home health, community support, and targeted case management services, or willingness to learn applicable state regulations and programs.
  • Strong skills in client assessment, care planning, coordination of services, and advocacy across interdisciplinary teams.
  • Proficient written and verbal communication skills, including clear documentation and professional interaction with clients, families, and providers.
  • Ability to manage time effectively, organize a caseload, meet deadlines, and work independently in a hybrid environment.
  • Comfort with basic technology, including electronic health or case management systems, email, and standard office software.
  • Commitment to cultural competency, respect for diversity, and trauma-informed, person-centered practices.
  • Relevant education such as an associate or bachelor’s degree in Social Work, Psychology, Human Services, Nursing, or a related field, or equivalent combination of experience and training.
  • Valid driver’s license, reliable transportation for community visits within Maine, and ability to meet COR Health background and compliance requirements.
