SUMMARY OF MAJOR FUNCTIONS

Under the direct supervision of the Behavioral Health Executive Director, the Behavioral Health Consultant/Integrated Care Therapist provides behavioral health consultation to children, adolescents, adults, and families in order to improve psychosocial functioning in the primary medical clinic. This requires coordination with the Clinical Services Administrator, Chief Medical Officer, and Behavioral Health Executive Director. 

 

Essential Duties AND Responsibilities

  1. Creates, develops, and nurtures culturally appropriate interactions and connections with staff, patients and the community.
  2. Provides brief interventions, assessments, and diagnoses of patients as a member of the Primary Care Team.
  3. Provides Motivational Interviewing SMART goal planning and assists clients in identifying and achieving goals.
  4. Evaluates crisis situations and applies appropriate interventions.
  5. Actively participates in meetings that support the Coquille Indian Tribe’s Integrated Health Care Model to provide holistic services.
  6. Assists in the detection of mental health disorders and provides appropriate recommendations and referrals.
  7. Assists the Primary Care Team in developing care management processes such as the use of screenings, guidelines, disease management techniques, case management, and psychoeducation to improve self-management of chronic disease.
  8. Accepts warm-handoffs in a fast-paced primary care clinic setting.
  9. Provides assessment, consultation, and brief intervention.
  10. Provides psychoeducation and interventions to promote desired health outcomes.
  11. Establishes and maintains professional relationships with other departments and services within the organization to ensure coordination of care.
  12. Monitors the site’s Behavioral Health Program, identifies problems related to patient services, and makes recommendations for improvement.
  13. Other duties as assigned.

PHYSICAL REQUIREMENTS

Requires the ability to communicate verbally, repetitive movement of the wrists, hands and/or fingers, often requires walking, standing, sitting for extended periods of time, raising or lowering objects, stooping and occasionally requires lifting up to 25 pounds.  Work is generally performed in an office setting and occasionally outdoors. Work involves occasional exposure to high risk/dangerous situations, infectious disease, dust, fumes, and allergens. Will often be required to travel by automobile, commercial or private carrier.  Local travel is frequently required, statewide and national travel is occasionally required. The individual must perform the essential duties and responsibilities with or without reasonable accommodation efficiently and accurately without causing a significant safety threat to self or others.

 

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

Individuals must possess the following knowledge, skills and abilities or be able to explain and demonstrate that they can perform the essential functions of the job, with or without reasonable accommodation, using some other combination of skills and abilities.

 

  1. Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions and a basic understanding of integrated behavioral health models.  
  2. Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.
  3. Knowledge of community structure, values, culture, and language of the population served.
  4. Ability to complete written reports and brief, concise, documentation of mental health assessments, treatment plans, progress notes, case management, and discharge plans in both paper form and as part of the electronic health records system accurately and on time. 
  5. Ability to consult and collaborate with other Tribal health, education, juvenile, justice, and social service staff in the development and implementation of treatment activities for youth and adults. 
  6. Ability to multitask, be flexible and adaptable in a clinic setting.
  7. Ability to plan and organize work and complete written reports and accurately and timely documentation of mental health assessments, treatment plans, progress notes, case management, and discharge plans in both paper form and as part of the electronic health records system. 
  8. Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong clinical communication skills.
  9. Good knowledge of psychopharmacology 
  10. Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.
  11. Knowledge of medical terminology
  12. Self-motivated and able to work independently and collaboratively in a group as needed
Qualifications:

REQUIRED Qualifications

  1. Master’s degree in Social Work, Psychology or Counseling, and Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Mental Health Counselor (LMHC) OR an equivalent licensure to the State of Oregon and ability to obtain Oregon licensure within six (6) months of hire. Health care background and experience preferred.
  2. Two (2) years of relevant experience is required. 
  3. A combination of formal education, training, and related work experience will be considered. 
  4. Basic computer proficiency, including Microsoft 365 applications and electronic communication tools.
  5. Knowledge of HIPAA privacy and confidentiality guidelines is required.
  6. Experience working within a Tribal environment is preferred.
  7. Experience with Electronic Health Records required.
  8. Current and valid Oregon driver’s license in good standing with no insurability issues as determined by the Tribe’s insurance carrier is required.
  9. Required to accept the responsibility of a mandatory reporter of abuse and neglect of infants and children, people who are elderly or dependent, individuals with mental illness or development disabilities or residents of nursing homes and other health care facilities. This includes reporting any evidence of physical injury, neglect, sexual or emotional abuse or financial exploitation.

 

 

Preferred Qualifications

  1. Knowledge of integrated behavioral health concepts and models preferred.
  2. Experience with PCBH model of integration preferred.

 

Salary : $80,600 - $100,734
