The Resident Care Coordinator is responsible for supporting the health, safety, comfort, and overall well-being of Copper Summit Assisted Living residents. This position provides hands-on resident care, coordinates daily care needs, administers medications in accordance with applicable regulations and facility policies, and works closely with residents, families, caregivers, and the nursing team.

The ideal candidate is compassionate, dependable, organized, and comfortable taking initiative in a fast-paced assisted living environment. This position plays an important role in ensuring residents receive consistent, person-centered care while maintaining accurate documentation and communication.

Required Qualifications

  • Current Med Pass Certification — REQUIRED
  • Current CPR Certification — REQUIRED
  • Previous experience in assisted living, senior care, medication administration, or a related healthcare setting preferred
  • CNA certification is a plus, but is not required
  • Must demonstrate the ability to safely and accurately administer medications according to physician orders, facility policies, and applicable regulations
  • Strong communication and organizational skills
  • Ability to work effectively as part of a team
  • Dependable, compassionate, and professional demeanor
  • Ability to maintain confidentiality and respect resident dignity and privacy

Important: Applicants must possess a current Med Pass Certification to be considered for this position. CNA certification does not replace the Med Pass requirement.

Key ResponsibilitiesResident Care

  • Provide compassionate, respectful, and person-centered care to residents
  • Assist residents with activities of daily living as needed, including bathing, dressing, grooming, toileting, mobility, and personal hygiene
  • Monitor residents for changes in condition, behavior, appetite, mobility, or overall well-being
  • Report changes or concerns promptly to the appropriate nursing personnel
  • Promote resident independence, dignity, choice, and quality of life
  • Respond appropriately to resident needs, emergencies, and call systems

Medication Management

  • Accurately administer medications in accordance with physician orders, facility policies, and applicable regulations
  • Complete medication passes safely and efficiently
  • Monitor residents for medication-related concerns or changes in condition
  • Accurately document medication administration and related observations
  • Communicate medication concerns, missed doses, refusals, or adverse reactions to the appropriate nursing personnel
  • Maintain proper medication security, storage, and handling procedures
  • Assist with medication ordering, refills, and pharmacy communication as assigned

Care Coordination & Documentation

  • Help coordinate daily resident care and ensure assigned care needs are addressed
  • Maintain accurate and timely resident documentation
  • Follow individualized service plans and care instructions
  • Communicate effectively with caregivers, nurses, physicians, pharmacies, residents, and families
  • Participate in care planning and resident-related meetings as assigned
  • Assist with maintaining accurate records related to resident care, medications, incidents, and observations

Resident & Family Relations

  • Build positive, respectful relationships with residents and their families
  • Respond to resident and family questions or concerns with professionalism and compassion
  • Help create a welcoming environment where residents feel safe, valued, and heard

Safety & Compliance

  • Follow all facility policies, procedures, safety standards, and applicable state and federal regulations
  • Maintain infection prevention and control practices
  • Complete required training and continuing education
  • Report accidents, incidents, injuries, abuse, neglect, or other safety concerns immediately according to facility policy
  • Maintain a clean, safe, and comfortable environment for residents

Physical Requirements

  • Ability to stand and walk for extended periods
  • Ability to assist residents with mobility and transfers as appropriate to training and facility procedures
  • Ability to bend, stoop, reach, push, pull, and lift as required to perform job duties
  • Ability to respond quickly to resident needs and emergencies
  • Ability to perform essential job functions with or without reasonable accommodation
