Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and comfort.
• Take and record vital signs including blood pressure, temperature, pulse, and respiration rates accurately using electronic health record (EHR) systems.
• Support medication administration under the supervision of nursing staff, ensuring proper documentation in medical records while adhering to HIPAA regulations.
• Help with patient transfers using equipment like Hoyer lifts to ensure safety for both patients and staff.
• Observe and report changes in patient condition or behavior promptly to the nursing team.
• Maintain a clean and safe environment by following aseptic techniques during procedures such as peri-care care
• Provide emotional support and companionship to patients, including those with dementia or memory care needs, fostering a caring atmosphere that enhances recovery.

Skills

• Proven experience working with diverse patient populations including geriatrics, pediatrics, or individuals with developmental disabilities.
• Familiarity with electronic health records (EHR/EHR systems) like PCC.
• Ability to perform tasks requiring heavy lifting and safe manual handling practices while maintaining patient safety standards.
• Strong communication skills to collaborate effectively with healthcare teams in fast-paced environments like emergency medicine or urgent care clinics.
• Experience with specialized care such as Alzheimer's care, hospice support, or behavioral health is highly valued.
• Commitment to maintaining HIPAA compliance and confidentiality when managing sensitive medical records and patient information.
